2型糖尿病患者心脏变时功能不全与糖脂代谢的相关性研究 被引量:4

Relationship between chronotropic incompetence and glucose and fat metabolism in patients with type 2 diabetes mellitus

摘要 目的探讨T2DM患者心脏变时功能不全(CI)与糖脂代谢的关系。方法 138例T2DM患者分为CI组和变时功能正常组(Con),比较两组糖脂代谢指标、血尿酸(SUA)、T2DM慢性并发症、心率变异(HRV)指标,包括RR间期标准差(SDNN)和相邻RR间期之差的均方根(rMSSD)。结果CI组2 hPG、HbA_1c、TC、TG和SUA高于Con组(t=2.067、3.640、3.699、2.120、3.740,P<0.05);CI组DPN患病率高于Con组(x^2=8.067,P<0.05);CI组SDNN、rMSSD较Con组降低(t=-8.157、-2.149,P<0.05)。多元Logistic回归分析结果显示,HbA_1c、TC、SUA和SDNN为CI的独立影响因素(OR=1.242、1.734、1.006、0.914,P<0.05)。结论长期、慢性的高血糖、高血脂可能是T2DM患者发生CI的重要因素,同时合并CI患者HRV降低。 Objective To explore the relationship between chronotropic incompetence (CI) and glucose and fat metabolism in patients with type 2 diabetes mellitus (T2DM). Methods 138 patients with T2DM were divided into CI group and normal group (Con). Compared to the two groups of glucose and lipid metabolism indicators, serum uric acid (SUA), T2DM chronic complications, heart rate variability (HRV) indicators,including the RR interval standard deviation (SDNN) and the difference between the adjacent RR interval (rMSSD). Results The levels of 2 hPG, HbA1 c, TC, TG, and SUA of CI group were significantly higher than Con group (t = 2. 067, 3. 640, 3. 699, 2. 120, 3. 740, P〈 0. 05). The prevalence of diabetic peripheral neuropathy(DPN)in CI group was significantly higher than Con group(X2 = 8. 067, P〈0. 05). SDNN and rMSSD of CI group were significantly lower than COn group(t =- 8. 157, --2. 149,P〈0.05). Multiple logistic stepwise regression analysis showed that HbAlc, TC, SUA, and SDNN were independent influence factors of CI (OR= 1. 242, 1. 734, 1. 006, and 0. 914, P〈0. 05). Conclusion Long-term and chronic hyperglycemia and hyperlipidemia may be important reasons of CI in T2DM patients with features of reduced HRV.
