摘要
阐述彭培初教授辨治肾病水肿的学术思想和治疗特色。在病因病机上重视外邪袭肺、脾不升清、肾阳亏虚、三焦壅滞、水道不通和风、气、水、瘀的致病作用及相关互病,注重疾病从表到里、从上到下、从实到虚、从气到血的动态变化过程;在治疗上常用清肺、升脾阳、温肾阳、疏调三焦、祛风除湿和逐水药及虫类药物,因势利导、内外分消,将水湿之邪从腠理和前后二窍排除体外,恢复气血津液的正常运行而水肿得愈。
This paper introduced Professor Peng Peichu＇s academic thoughts and clinical experience in the treatment of renal edema. In the etiology and pathogenesis,it emphasizes the effects of exogenous factors attacking the lungs,spleen failing to rise,kidney yang deficiency,obstruction of triple energizer and water passage and interaction of wind,qi,water and blood stasis,and pays attention to the dynamic process of the disease： from outside to inside,from top to bottom,from excess to deficiency,and from qi to blood. In the treatment,prescriptions of clearing lung,up-lifting spleen yang,warming kidney yang,unblocking triple energizer,dispersing wind and dampness,as well as water-discharging herbs and insects drugs,are commonly used,which adroitly guide actions according to circumstances and consequently remove water and dampness from the skin and lower orifices to recover the normal circulation of qi-blood-body fluid.
