摘要
目的观察地衣芽孢杆菌活菌胶囊在根除幽门螺杆菌治疗中的应用。方法选取2013年1月。2016年1月我院内科门诊就诊中幽门螺杆菌检测阳性的患者126例,采用分层随机化分组为A组、B组与C组,每组均42例。A组用三联疗法(奥美拉唑20mg,bid+阿莫西林0.1g,bid+克拉霉素0.5g,bid),B组用四联疗法(枸橼酸铋钾胶囊0.6g,bid+三联疗法),c组用地衣芽孢杆菌活菌胶囊0.5g,dd+四联疗法,每组疗程均为14d。疗程结束后至少间隔4周行”c尿素呼气实验检查治疗效果,治疗期间、治疗后均记录不良反应发生情况。结果幽门螺旋杆菌根除率分别为A组66.7%,B组88.1%,c组92.9%。B、C组疗效明显高于A组(P〈0.05)。A、B、C三组不良反应发生率分别为19.1%、23.8%、4.8%,C组不良反应发生率明显低于A、B组(P〈0.05)。结论含有铋剂的四联疗法根除幽门螺旋杆菌疗效明显高于标准三联疗法;地衣芽孢杆菌活菌胶囊加四联疗法虽较四联疗法对幽门螺旋杆菌根除率提升不明显,但能显著减少治疗过程中不良反应发生率。
Objective To observe the role of the bacillus licheniformis capsu!e in treatment of Helicobacter pylori eradication. Methods 126 patients of Helicobacter pylori positive from January 2013 to January 2016 in our hospital were divided into group A, group B and group C, 42 patients in each group with stratified randomization. Group A received triple combination therapy (omeprazole, a'moxicillin and clarithromycin), Group B received quadruple combination therapy (bismuth potassium citrate plus triple combination therapy), Group C received quadruple combination therapy and bacillus licheniformis capsule, each group of patients were treated for 14 days. At least 4 weeks after the end of the course of treatment, all patients had to examine 13C urea breath test to prove,the treatment efficacy. Adverse reactions were recorded during and after treatment. Results Hp eradication rate of group A was 66.7.%, group B was 88.1% and group C was 92.9%. the treatment efficacy of group B and C were both higher than group A, the difference was statistically significant(P 〈 0.05). The adverse effects rate of group A was 19.1%, group B was 23.8% and group C was 4.8%, the adverse effects rate of group C was lower than group A andB, the difference was statistically significant(P 〈 0.05). Conclusion treatment efficacy of Hp eradication of bismuth-based quadruple combination therapy is significantly higher than standard triple combination therapy. The difference of Hp eradication rate have no statistical significance between bacillus licheniformis capsule plus bismuth-based quadruple combination therapy and simple quadruple combination therapy, but the bacillus licheniformis capsule can significantly reduce the adverse reactions of bismuth-based quadruple combination therapy.
