3D打印结合定制钢板内固定技术在骨盆骨折治疗中的应用 被引量:23

Application of three-dimensional printing combined with individualized custom-made plate in internal fixation of pelvic fractures

摘要 目的探讨3D打印结合定制钢板治疗骨盆骨折的临床疗效。方法采用回顾性队列研究分析2011年2月-2015年6月收治的91例新鲜闭合骨盆骨折患者,按随机数字表法分为3D打印结合定制钢板内固定组(3D组)和单纯重建钢板内固定组(传统组)。3D组42例,其中男29例,女13例;年龄(38.4±7.2)岁。传统组49例,其中男37例,女12例;年龄(39.1±6.8)岁。根据Tile分型:B型35例,C型56例。3D组和传统组受伤至手术时间分别为(6.5±2.2)d和(5.1±1.7)d。3D组术前通过3D打印技术完成骨盆骨折三维建模并打印实物模型,根据三维建模数据定制出个体化钢板;传统组则按常规方式完成术前计划,采用重建钢板内固定。比较两组手术时间、术中出血量、围术期输血量、术中透视次数、骨折愈合时间、恢复正常活动时间、不良事件发生率。采用MaLta标准评估复位质量,术后1年采用Majeed标准进行功能评分。结果3D组与传统组术后随访时间分别为(21.2±3.7)个月和(26.3±3.9)个月。3D组手术时间、术中出血量、围术期输血量、术中透视次数均较传统组缩短或减少[(78.2±11.7)min:(93.6±12.1)min、(452.7±102.1)ml:(723.3±147.5)ml、(6.9±3.2)U:(11.3±4.6)U、(4.2±1.2)次:(7.3±1.1)次](P〈0.05)。3D组较传统组术后恢复正常活动时间早[(12.5±0.9)周:(14.1±3.6)周](P〈0.05),不良事件发生率低(5%:8%)(P〈0.05)。3D组Matta标准及Majeed标准评分优良率均高于传统组(93%:88%、90%:86%)(P〈0.05)。两组骨折愈合时间比较差异无统计学意义(P〉0.05)。结论3D打印结合定制钢板内固定技术治疗骨盆骨折可缩短手术时间、提高骨折复位质量、减少手术创伤和手术风险,可为患者提供个体化治疗,有效改 Objective To investigate the clinical efficacy of three-dimensional (3D) printing combined with individualized custom-made plate in fixation of pelvic fractures. Methods Ninety-one patients with fresh closed pelvic fractures hospitalized from February 2011 to June 2015 were analyzed retrospectively. The patient population was assigned into two groups according to the random number table : 3D group ( 29 males and 13 females) aged ( 38.4 ±7.2) years were treated using the 3 D-printing combined with individualized custom-made plate internal technique and traditional group (37 males and12 females) aged (39. 1 ±6.8)years were treated using the traditional methods. Tile classification of pelvic fractures was type B in 35 patients and type C in 56 patients. Time interval from injury to operation was (6.5±2.2) days in 3D group and (5.1 ±1.7)days in traditional group. Operation time, intraoperative bleeding, blood transfusion, fluoroscopy frequency, bone union time, recovering time and adverse event rate of the two groups were recorded. Quality of fracture reduction was evaluated using the Matta criteria and functional restoration was assessed at postoperative 1 year using the Majeed criteria. Results Follow- up time was ( 21.2 ±3.7 ) months in 3D group and ( 26.3± 3.9 ) months in traditional group.
