摘要
目的 探究伴跟骰关节损伤的SandersⅡ型及Ⅲ型跟骨骨折运用切开复位跨跟骰关节锁定钛合金钢板内固定法干预的疗效观察。方法 选取2009年6月至2014年4月期间本院收治的单侧伴跟骰关节损伤的SandersⅡ型及Ⅲ型跟骨闭合性骨折患者40例,采用随机分组方式,将其分为对照组与实验组,各20例。针对对照组,采用切开复位不跨关节钢板结合克氏针内固定,针对实验组,采用切开复位跨跟骰关节锁定钢板固定方法,对比分析两组患者治疗效果、术后生活质量以及手术并发症情况。结果 （1）基于跟骨Maryland足部评价方法,对比两组患者治疗效果,实验组优良率明显高于对照组,差异显著,具有统计学意义（P〈0.05）;（2）以生活质量评价标准为依据,对比两组患者术后9~18个月生活质量,实验组与对照组无明显差异（P〉0.05）;（3）通过观察得知,实验组手术并发症明显低于对照组,差异显著,具有统计学意义（P〈0.05）。结论 针对伴跟骰关节损伤的SandersⅡ型及Ⅲ型跟骨骨折患者,通过切开复位跨跟骰关节锁定钛合金钢板内固定法进行治疗,效果显著,值得临床医学广泛应用。
Objective To explore the curative effect of open reduction and internal fixation with titanium plate cross calcane-ocuboid joint in the treatment of Sanders type II and III calcaneal fractures with calcaneocuboid joint injury. Methods Admitted into our hospital from June 2009 to April 2014, 40 patients with Sanders type II and III closed calcaneal fractures with unilateral calcane-ocuboid joint injury were selected and randomly divided into control group and experiment group with 20 cases in each group. The con-trol group was treated with open reduction and internal fixation with plate and Kirschner wire uncrossing calcaneocuboid joint, while the other group was given open reduction and internal fixation with titanium plate cross calcaneocuboid joint. The curative effect, postoper-ative life quality and complications of the two groups were compared and analyzed. Results （1） According to Maryland calcaneal e-valuation system, the effect of the experiment group was remarkably better than that of the control group, with statistically significant difference （P〈0. 05）. （2） According to the comparison of life quality, 9 to 18 months after operation, no significant difference was found in two groups （ P〉0. 05 ） . （ 3 ） The observation showed that operation complications in the experiment group were significantly less than the control group, with statistically significant difference （P〈0. 05）. Conclusion The method of opening reduction and in-ternal fixation with titanium plate cross calcaneocuboid joint has remarkable curative effect on Sanders type II and III calcaneal fractures with calcaneocuboid joint injury, so it is worth promoting.
