摘要
目的:分析应用无创呼吸机治疗慢性阻塞性肺疾病(慢阻肺)合并肺源性心脏病患者的临床效果.方法:纳入我院收治的慢阻肺合并肺源性心脏病患者72例,均于2013年7月~ 2015年9月期间就诊,按入院顺序编号后随机分组,即对照组(常规治疗)和观察组(常规疗法联合无创呼吸机治疗)各36例,对比两组的治疗效果和相关指标水平变化情况.结果:观察组的PaCO2、HCO3-、肺动脉压力分别为(46.58 ±7.92) mmHg、(31.83±5.29) mmol/L、(41.48±4.59) mmHg,均低于对照组,且观察组的PO2、FEV1、FEV1/FVC分别为(67.81 ±6.49) mmHg、(75.38±3.10)%、(56.27±4.81)%,较对照组更高,组间对比差异显著(t=15.39,12.90,13.28,13.78,15.98,15.28;P<0.05);且治疗后观察组的Nt-proBNP水平较对照组更低(t=13.67,P<0.05).结论:应用无创呼吸机对慢阻肺合并肺源性心脏病患者予以治疗,能够在改善患者呼吸功能的同时,促进心功能恢复,降低心衰发生风险,临床应用价值高.
OBJECTIVE: To analyze the effect of noninvasive ventilator in the patients with chronic obstructive pulmonary disease (COPD) combined with cor pulmonale. METHODS: 72 patients with COPD combined with cor pulmonale from July 2013 to Septem- ber 2015 in our hospital were divided into the control group (n = 36, routine treatment) and observation group (n = 36, conventional therapy in combination with noninvasive ventilator therapy). Treatment effect arid ralated index level changes were compared in two groups. RESULTS: PaCO2 (31.83 ±5.29) mmol/L, HCO3 - (41.48±4. 59) mmHg and pulmonary artery pressure (46. 58 ±7.92) mmHg in the observation group were lower than those in the control group. PO2 (75.38± 3.10)%, FEV1 (56.27± 4. 81 )% and FEVJFVC (67.81±6.49) mmHg in the observation group were higher than those in the control group. There was the significant difference between two groups ( P 〈 0.05 ). After treatment in the observation group Nt - pro BNP levels were lower than those in the control group (P 〈 O. 05 ). CONCLUSION : The application of noninvasive ventilator treatment in the patients with COPD combmed with cot pulmonale can improve the patients' respiratory function, promote the heart function recovery, and reduce the risk of developing heart failure.
