中药注射剂的药物相互作用研究进展 被引量:10

摘要 通过检索近年来文献,并结合相关法规要求和药品说明书现状,对中药注射剂药物相互作用的相关报道和临床应用现状进行综述,以了解中药注射剂药物相互作用的研究现状,为临床安全合理用药及相关领域的研究提供参考。结果发现中药注射剂药品说明书中关于药物相互作用的内容几近空白;研究报道显示,有些中药注射剂与其他药物联用时可发生药物相互作用,可能使药效增强或减弱,其作用机制包括CYP450酶介导、影响药物与血浆蛋白结合、影响药物在肾脏的重吸收等,以及影响药效学。中药注射剂药品说明书中关于药物相互作用的内容亟需完善;中药注射剂与其他药物联用时可产生药动学和药效学药物相互作用,但相关领域的研究尚很欠缺,需进一步广泛深入开展;临床用药时应关注中药注射剂的药物相互作用问题,保障中药注射剂的安全合理使用。 By summarizing recent studies, related legal requirements and drug instructions, we assessed the current clinical application of traditional Chinese medicine injections-based drug-drug interactions to determine traditional Chinese medicine injections-based drug-drug interactions and provide reference for rational drug use. The result showed that there was almost no description on drug-drug interactions in the instructions of traditional Chinese medicine. The literature reports showed that drug-drug interactions would occur if traditional Chinese medicine injections were used in combination with other drugs, resulting in increased or decreased curative effects. The mechanisms included intervention of CYP450, influence on combination between drugs and plasma proteins, influence on re-absorption of drug in the kidney and pharmacodynamic influence. There is urgent need to improve the instructions for traditional Chinese medicine drug-drug interactions. Pharmacokinetic and pharmacodynamic interactions may occur when traditional Chinese medicine injections are used in combination with other drugs. Attention should be paid to the drug-drug interactions, to ensure the safety and reasonable drug use.
