摘要
目的:探讨杂交手术治疗下肢动脉硬化闭塞症并血栓形成的疗效。方法:回顾分析我科2012年1月-2016年2月收治的39例下肢动脉硬化闭塞症并血栓形成患者,病变均累及髂动脉,均行全麻下杂交手术治疗,先尝试患侧股总动脉切开双向取栓,其中髂动脉球囊扩张并支架置入39例,股浅动脉球囊扩张并支架植入10例,膝下3分叉球囊扩张4例,股浅动脉病变旷置未处理3例。结果:39例患者术后发凉、疼痛症状消失,间歇性跛行缓解,踝肱指数(ABI)值平均升高至0.91±0.10;坏疽趾头截趾10例,一期愈合;出院后随访1-4年,其中死亡5例,股浅动脉血栓复发3例,股浅动脉支架再狭窄2例。结论:杂交手术是治疗下肢动脉硬化闭塞症并血栓形成的有效方法。
Objective：To probe into the effect of hybrid operation for arteriosclerosis obliterans and thrombosis treatment.Methods：We retrospectively analyzed 39 cases of arteriosclerosis obliterans and thrombosis with iliac artery damaged in our department from Jan.2012 to Feb.2016.All cases underwent hybird surgical treatment under general anesthesia.All common femoral artery in diseased side were executed bi-directional incision embolectomy,39 cases of iliac artery were treated with balloon dilation and stent placement;10cases of superficial femoral artery were treated with balloon dilation and stent placement;4cases of tibia and fibual artery were treated with balloon dilation;3cases of superficial femoral artery lesion could be ignored.Results：39cases with symptoms such as numbness,coldness,pain,claudication were relieved after operation,ABI increased to 0.91±0.10.10cases′gangrene toe were amputated with primary healing.All patients were followed up for 1-4years：5of them dead,3thrombosis and 2restenosis recurring in SFA.Conclusion：Hybrid operation can provide a feasible and effective treatment for patient with arteriosclerosis obliterans and thrombosis.
