摘要
目的:研究急性心肌梗死(AMI)相关血管与QT离散度(QTd)的关系。方法:选择2012年3月~2014年3月间在我院完成冠状动脉造影且成功进行急诊经皮冠状动脉介入治疗(PCI)的患者143例,将其设为PCI组,根据血管病变部位将其分为右冠脉组(RCA组,40例)、左回旋支组(LCX组,47例)、左前降支组(LAD组,56例);选择同期在我院确诊AMI,未行急诊PCI治疗的患者78例为AMI对照组。记录各组患者在入院时、入院后24h、48h的12导联心电图,比较各组的QTd及QT间期(QTcd)。结果:与入院时比较,AMI对照组和PCI组入院24h和48h后QTd和QTcd显著缩短(P〈0.05或〈0.01);与AMI对照组比较,PCI组QTd[24h:(56.82±11.47)ms比(32.90±2.19)ms,48h:(47.85±10.74)ms比(31.64±2.73)ms]和QTcd[24h:(60.49±12.40)ms比(40.06±4.68)ms,48h:(58.17±13.38)ms比(38.42±3.29)ms]降低更显著,P均〈0.01。与入院时比较,入院24h和48h时LAD组、LCX组和RCA组QTd和QTcd显著降低(P均〈0.01),但三组间无显著差异(P均〉0.05)。结论:对急性心肌梗死患者进行急诊介入治疗后,患者QT离散度及QT间期有明显改善,而不同病变血管对心室肌复极的影响程度无明显差异。
Objective:To study the relationship among related vessels of acute myocardial infarction(AMI)and QT dispersion(QTd).Methods:A total of 143 patients,who received coronary angiography and emergency percutaneous coronary intervention(PCI)in our hospital from Mar 2012 to Mar 2014,were enrolled as PCI group.According to vascular lesion sites,they were divided into right coronary artery(RCA)group(n=40),left circumflex(LCX)group(n=47)and left anterior descending(LAD)group(n=56);another 78 cases diagnosed as AMI without emergency PCI treatment were selected as AMI control group in our hospital simultaneously.The 12-lead ECG was recorded in each group at hospitalization,24 hand 48h after hospitalization,then QTd and QT interval(QTcd)were compared among all groups.Results:Compared with at hospitalization,on 24 hand 48h after hospitalization,there were significant reductions in QTd and QTcd in AMI control group and PCI group(P〈0.05 or0.01);compared with AMI control group,there were significant reductions in QTd [24h:(56.82±11.47)ms vs.(32.90±2.19)ms,48h:(47.85±10.74)ms vs.(31.64±2.73)ms]and QTcd [24h:(60.49±12.40)ms vs.(40.06±4.68)ms,48h:(58.17±13.38)ms vs.(38.42±3.29)ms]in PCI group,P〈0.01 all.Compared with at hospitalization,on 24 hand 48h after hospitalization,there were significant reductions in QTd and QTcd in LAD,LCX and RCA group(P〈0.01 all),but there were no significant difference among three groups,P〈0.05 all.Conclusion:QT dispersion and QT interval significantly improve in AMI patients after emergency PCI,but there is no significant difference in influence on ventricular myocardial repolarization among different diseased vessels.
