摘要
目的:探讨改进的股骨近端防旋髓内钉(PFNA)进行微创治疗股骨粗隆间骨折的方法及疗效。方法:回顾分析重钢总医院2011年7月至2015年10月37例股骨粗隆间骨折患者。按照Evans—Jensen分型,IA型3例,IB型5例,ⅡA型11例,ⅡB型9例,Ⅲ型9例,所有患者均采用闭合复位PFNA内固定术治疗。分析患者的手术时间、术中出血量,随访骨折愈合情况,采用Harris功能评分评估患髋功能。结果:治疗期间无医疗事故发生,37例患者均获随访,患者骨性愈合良好,随访时间3~24个月,平均12个月。Harris功能评分优21例,良12例,可4例,总体优良率89.1%。结论:PFNA微创治疗股骨粗隆间骨折是理想的手术方式,操作简单、创伤小、出血少、手术时间短,螺旋刀片具有很强的抗切出能力,可早期下床活动。
Objective Discussion Improved proximal femoral nail anti-rotation methods and efficacy of minimally invasive treatment of intertrochanteric fracture. Methods Retrospective analysis of 37 cases of intertrochanteric fracture patients, 14 cases of male, female 23 cases; Aged 30 ~ 91, the average at the age of 61. According to the Evans-Jensen parting, type IA in 3, the IB type 5 cases, Ⅱ A type A 11 cases, Ⅱ type B 9 cases, Ⅲ type 9 cases, all patients were treated with closed reduction and PFNA fixation. Analysis of patients with operation time, intraoperative blood loss, follow-up of fracture healing, functional assessment score by Harris hip function. Results 37 patients have been followed up, follow-up of 4 ~ 24 months, an average of 12 months. This group of patients with surgical time is 45 ~ 135 min, an average of 71.5 min, intraoperative blood loss of 50 ~ 120 ml, an average of 88 ml. Clinical healing time was 2.8 ~ 4.2 months, an average of 3.6 months. Harris functional score 21 cases, 12 cases, 4 cases, the total was 89.1%. Conclusion PFNA minimally invasive treatment of intertrochanteric fractures is an ideal operation method, simple operation, small trauma, less bleeding, shorter operation time, the spiral blade has a strong ability to resist cut out, but early ambulation.
