摘要
目的 探讨微创经皮椎弓根钉内固定（PPSF）治疗胸腰椎压缩性骨折（TLVCF）的临床效果。方法 选择2012年6月～2015年6月进贤县人民医院收治的TLVCF患者84例,根据治疗方法不同分为开放组和微创组,每组42例,分别采取开放与PPSF治疗。比较两组的治疗效果。结果 微创组手术时间、切口长度、术中出血量及住院天数均显著少于开放组（P〈0.05）;微创组术后视觉模拟评分评分和Oswestry功能障碍指数明显低于开放组（P〈0.05）;两组伤椎椎体前高比、矢状面后凸Cobb角比较,差异均无统计学意义（P〉0.05）。结论 在治疗TLVCF方面,PPSF与传统开放手术具有相似的矫正效果,但微创PPSF创伤小、术后恢复快,有利于术后腰背部功能及疼痛的改善,值得推荐。
Objective To investigate the clinical effect of PPSF in the treatment of TLVCF.Methods Eighty-four patients with TLVCF in Jinxian County People′s Hospital from June 2012 to June 2015 were selected,and they were divided into open group and minimally invasive group according to the treatment methods,42 cases in each group,they were treated with open and percutaneous pedicle screw fixation respectively.The treatment effect of patients in the two groups was compared.Results The operation time,length of incision,amount of bleeding and length of stay of the minimally invasive group were significantly less than those of the open group（P〈0.05）.The VAS score and Oswestry disability index of the minimally invasive group were significantly lower than those of the open group（P〈0.05）.There was no significant difference between the two groups in the vertebral anterior height ratio and the sagittal plane Cobb′s angle（P〉0.05）.Conclusion In the treatment of TLVCF,PPSF and traditional open surgery with similar correction effect,but PPSF is less trauma,faster postoperative recovery,and conducive to the postoperative pain and improve the function of the back,it is worthy of recommendation.
