阿奇霉素对肺炎患儿胃动力的影响

摘要 目的探讨阿奇霉素对肺炎患儿胃电参数和胃肠激素的影响及二者之间的相关性。方法采用自身前后对照方法,观察肺炎患儿阿奇霉素治疗前后胃电参数和胃肠激素的变化。43例肺炎患儿治疗前后均于清晨空腹采血,采血半小时后采用XDJ-S8B型胃肠电图仪记录空腹胃电参数。利用酶联免疫法检测血清胃动素(MLT)、P物质(SP)、降钙素基因相关肽(CGRP)及血管活性肠肽(VIP)含量。结果①阿奇霉素治疗2 d后,肺炎患儿的胃电波形平均频率(F)增快(P<0.01),正常慢波百分比(PNSW)减少(P<0.01),而波形平均幅值(VPP)较治疗前差异无统计学意义(P>0.05)。②阿奇霉素治疗后,肺炎患儿血清MLT升高(P<0.05),SP含量升高(P<0.01),而VIP及CGRP较治疗前差异无统计学意义(均P>0.05)。③相关分析显示,VPP与SP呈正相关(P<0.01),与VIP呈负相关(P<0.01)。F与MLT呈正相关(P<0.01),与VIP呈负相关(P<0.05)。PNSW与VIP呈正相关(P<0.05)。结论阿奇霉素能加快肺炎患儿的胃电节律,降低正常慢波百分比,具有一定的促胃动力作用,也可能导致胃动力紊乱。阿奇霉素对胃动力的影响可能与血浆胃动素和P物质的增高有关。 Objective To explore the influences of azithromycin( AZI) on gastric electrical parameters and gastrointestinal hormones in children with pneumonia and their correlations. Methods Forty-three children with pneumonia were collected to observe the changes of gastric electrical parameters and gastrointestinal hormones before and after AZI treatment. Fasting plasma motilin,vasoactive intestinal peptide( VIP),substance P( SP) and calcitonin gene-related peptide( CGRP) were detected by ELISA. Gastric electrical activity signals were recorded by electrogastrography( EGG) half an hour after blood collection. Results ① After 2 d AZI treatment,the gastric electrical waveform average frequency( F) in children with pneumonia increased( P〈0. 01),the percentage of normal slow wave( PNSW) decreased( P〈0. 01),while the values of average amplitude( VPP) showed no different change( P〉0. 05). ②After AZI treatment,the motilin level in children with pneumonia increased( P〈0. 05),and SP level also increased significantly( P〈0. 01),while the levels of VIP and CGRP showed no different change( P〉0. 05). ③Correlation analysis showed that VPP was positively associated with SP( P〈0. 01),but negatively associated with VIP( P〈0. 01). The average frequency of the waveform was positively correlated with motilin( P〈0. 01),but negatively correlated with VIP( P〈0. 05). The PNSW was positively correlated with VIP( P〈0. 05). Conclusion AZI can speed up the gastric electrical rhythm in children with pneumonia,decrease the percentage of normal slow waves,which suggests that AZI may promote the gastric motility,but may also lead to gastric motility disorders. Higher plasma motilin and substance P levels maybe contribute to the effect of azithromycin on gastric motility.
