摘要
目的研究亚冬眠疗法对急性肺损伤(acute lung injury,ALI)早期呼吸功能改变和血清炎性因子表达的影响。方法选取30只8~10周龄健康雄性新西兰大白兔随机分为正常对照组(麻醉后不致伤,仅参照亚冬眠治疗组给予相同剂量的生理盐水注射)、ALI组(致伤后参照亚冬眠治疗组给予相同剂量的生理盐水注射)和亚冬眠治疗组(致伤后给予冬眠合剂腹腔注射并以维持量至实验结束)。于肺损伤模型建立后0、1、2、4、6 h记录生命体征、肺功能,并于上述时间点抽动脉血,检测血气及血清炎性因子[肿瘤坏死因子-a(TNF-a)、白细胞介素-1β(IL-1β)和C反应蛋白(CRP)]的表达。结果亚冬眠治疗后体温明显降低(P〈0.05),ALI组损伤后呼吸频率、心率明显增快(P〈0.05),血压降低(P〈0.05),亚冬眠治疗组变化幅度较小(P〈0.05)。ALI组肺潮气量和肺泡通气量伤后6 h明显低于亚冬眠治疗组(P〈0.05),而分钟通气量损伤后1 h达峰值,明显高于亚冬眠治疗组((P〈0.05)),亚冬眠治疗组生理死腔-潮气量在不同时间点均低于ALI组(P〈0.05);ALI组Pa O2、Sa O2和p H值明显降低(P〈0.05),Pa CO2明显增加(P〈0.05),亚冬眠治疗组变化幅度较小(P〈0.05);ALI组血清中TNF-α、IL-1β和CRP含量显著升高(P〈0.05),亚冬眠治疗在各个时间点均减低(P〈0.05)。结论在ALI早期给予亚冬眠治疗可明显减少血清炎性因子的表达,对损伤肺的通气功能、换气功能有明显的保护作用。
Objective To investigate effect of sub-hibernation therapy on changes of early respiratory function and expressions of serum inflammatory factors of rabbits with acute lung injury(ALI).Methods A total of 30 male healthy New Zealand rabbits with 8-10 weeks old were randomly divided into control group,ALI group and sub-hibernation therapy group(n = 10 for each group).Control group was injected with same dose of normal saline referred to the dose of sub-hibernation therapy group after anesthesia without being vulnerated; ALI group was injected with same dose of normal saline referred to the dose of sub-hibernation therapy group after anesthesia and being vulnerated; sub-hibernation therapy group was intraperitoneally injected with hibernation mixture with maintenance dose to the end of the experiment after anesthesia and being vulnerated.Vital signs and lung function were recorded at 0,1,2,4 and 6 h after establishment of lung injury models,and samples of arterial blood were collected at the same times to detect arterial blood gas index and expressions of serum inflammatory factors [tumor necrosis factor-a(TNF-a),interleukin-1β(IL-1β) and C-reactive protein(CRP) ].Results Animal body temperature decreased obviously after sub-hibernation therapy(P〈0.05).In ALI group,values of breathing rate and heart rate were significantly faster(P〈0.05),while value of blood pressure were decreased after being vulnerated(P〈0.05),and the changes by sub-hibernation therapy were less(P〈0.05).In ALI group,values of tidal volume and alveolar ventilation volume at 6 h after being vulnerated were significantly lower than those in sub-hibernation therapy group(P〈0.05),while the minute ventilation reached the peak at 1h after being vulnerated,and the peak value was significantly higher than that in sub-hibernation therapy group(P〈0.05).Values of physiological dead space-tidal volume in sub-hibernation therapy group at different time points were lower than those in ALI group(P〈0.05
