丁苯酞辅助治疗对急性脑梗死患者的临床效果 被引量:12

摘要 目的探讨丁苯酞辅助治疗对急性脑梗死患者血清NSE、MMP-9、SOD水平及侧支循环建立的影响。方法本研究自2013年3月-2015年4月收集在东阳市人民医院神经内科住院治疗的急性脑梗死患者90例,按随机数字表法将患者平均分为对照组和研究组,2组患者基线资料无统计学差异,对照组45例采用抗血小板、控制血压、营养脑神经、减轻脑水肿、神经康复等常规治疗,研究组45例在常规治疗上予以丁苯酞注射液辅助治疗,治疗前及治疗14d后抽血测定血液流变学、血清NSE、MMP-9、SOD水平及侧支循环指标,同时对比临床疗效及侧支循环代偿的改善情况。结果对照组有效率及侧支循环改善率分别为82.22%、11.11%,研究组有效率及侧支循环改善率分别为95.56%、42.22%,差异有统计学意义(P〈0.05);与对照组比,研究组治疗后血液粘度指标及红细胞压积降低,血清NSE、MMP-9水平降低,血清SOD水平升高,血清VEGF、BDNF、b FGF含量升高,具有统计学意义(P〈0.05)。结论丁苯酞辅助治疗可促进急性脑梗死患者侧支代偿血管建立,改善缺血半暗带血供进而改善神经功能缺损,效果显著。 Objective To investigate the effect of butylphthalide on NSE,MMP-9,SOD serum levels and collateral circulation establishmentin treatment of acute cerebral infarction. Methods Ninety patients diagnosed as acute cerebral infarction and met criterions were collected from March 2013 to April 2015 in our hospital. They were divided into two groups: experiment group and control group according to the different treatment methods. General information of the two groups were comparable. Patients in the control group received conventional therapy,such as anti-platelet,blood pressure control,nutrition,brain,brain edema,neurological rehabilitation. The patients in the experiment group received butylphthalide injection on the base of routine treatment. Measured the hemorheology,serum NSE,MMP-9 and SOD levels and collateral circulation indexes before treatment and after treatment of 14 d,compared the clinical curative effect and collateral circulation compensation improvement situations. Results The efficiency rate and collateral circulation improvement rate of the control group were 82. 22% and 11. 11%. The efficiency rate and collateral circulation improvement rate of the experiment group were 95. 56% and 42. 22%. The difference was statistically significant( P〈0. 05). Compared with control group,the blood viscosity indexes and hematocrit of the experiment group were decreased after treatment,the serum NSE and MMP-9 levels were decreased,the SOD level was increased,the serum VEGF,BDNF,b FGF contents were increased with statistical significance( P〈0. 05). Conclusion Butylphthalide could promote the establishment of collateral vessels,improve the blood supply of ischemic semi dark blood supply and improve the neurological function defect in the treatment of patients with acute cerebral infarction.
