摘要
目的系统评价膳食添加益生菌、益生元、合生元对2型糖尿病患者血糖、血脂与应激状态的影响。方法检索MEDLINE、EMBASE、Cochrane对照试验中心注册数据库(cochrance central register of controlled trials,CENTRAL),检索语言为英语,检索起止时间为2000年至2015年。检索词为:type2diabetes,prebiotics。纳入对糖尿病患者添加益生菌、益生元、合生元的随机对照试验,采用Jadad量表进行研究质量评价,使用RevMan 5.2软件进行Meta分析。结果最终纳入的6项来自2个不同国家的高质量研究。试验组患者空腹血浆葡萄糖水平(I2=61%,95%CI=-41.10^-10.27,Z=-25.67,P=0.001)、甘油三酯(I2=40%,95%CI=-54.61~14.86,Z=-34.74,P=0.0006)、超敏C反应蛋白CRP(I2=0%,95%CI=-8.68^-4.52,Z=-6.6,P<0.00001)低于对照组,差异具有统计学意义。糖化血红蛋白(I2=71%,95%CI=-0.80~0.23,Z=1.08,P=0.28)、胆固醇(I2=78%,95%CI=-39.87~3.12,Z=-18.37,P=0.09)在两组之间差异无统计学意义。结论膳食补充益生菌、益生元、合生元可以改善2型糖尿病患者血糖、血脂状况,降低应激反应。
Objective To systematically evaluate the effects of probiotics,prebiotics and synbiotics on blood glucose,lipid,and state of stress among patients with type 2diabetes.Methods The databases of MEDLINE,EMBASE and Cochrane were retrieved for literatures in English published from 2000 to 2015,with type 2diabetes and prebiotics as keywords.All the randomized controlled trials were included,and were assessed by using Jadad scale.RevMan 5.2was used for statistical analysis.Results 6trials from two countries were included at last.The levels of fasting blood glucose（I2=61%,95%CI=-41.10--10.27,Z=-25.67,P=0.001）,triglycerides（I2=40%,95%CI=-54.61-14.86,Z=-34.74,P=0.0006）,and CRP（I2=0%,95%CI=-8.68--4.52,Z=-6.6,P〈0.00001）in experimental groups were lower than those in control groups,with statistically significant differences.However,there had no statistical differences between two groups in the levels of glycosylated hemoglobin（I2=71%,95%CI=-0.80-0.23,Z=1.08,P=0.28）and cholesterol（I2=78%,95%CI=-39.87-3.12,Z=-18.37,P=0.09）.Conclusion Supplementary probiotics,prebiotics and synbiotics can improve the levels of blood sugar and blood lipid of patients with type 2diabetes,and decrease oxidative stress.
