摘要
目的了解布鲁氏菌病急性期患者常见感染性指标、体液免疫检查结果与其关节、肌肉疼痛程度的相关性。方法采用回顾性分析方法对某医院2013年1月-2015年7月期间54例（其中轻症43例,重症11例）检查项目相对完整的布鲁氏菌病急性期患者的临床数据进行分析,比较轻症与重症患者之间一般实验室感染指标和体液免疫检查结果的差异以及病情缓解与住院时间平均值的差异。结果轻症与重症组患者的外周血白细胞计数、红细胞沉降率、降钙素原及体液免疫（Ig M、Ig G、Ig A、C3和C4）等结果差异均无统计学意义（t=1.94、1.87、1.64、1.25、1.16、1.59、1.47和1.61,所有P〉0.05）。重症组患者C-反应蛋白值为（48.5±1.9）mg/L,显著高于轻症组患者的（11.4±1.6）mg/L,差异有统计学意义（t=2.24,P〈0.05）;重症组患者疼痛缓解平均时间长于轻症组,两组分别为（20.5±1.6）d和（12.4±1.7）d,差异有统计学意义（t=2.25,P〈0.05）;重症组患者住院时间长于轻症组,两组平均值分别为（21.2±3.7）d和（14.6±3.3）d,差异有统计学意义（t=2.37,P〈0.05）。结论布鲁氏菌病急性期患者关节、肌肉疼痛较重,患者的C-反应蛋白值升高明显,住院时间较长,提示炎症较重,临床亟需改善他们的症状。
Objective To survey the difference of common laboratory indexes of infectivity and humoral immunity in acute pa- tients, who infected brucellosis with different level of pain of arthrosis and muscle tissue. And select which items are correlation with the level of pain. Methods Fifty four actue patients which infected with brucellosis were reviewed, who were admitted in our hospital between January 2013 to July 2015 and have relative complete common laboratory indexes of infectivity and humoral immunity, then divided into two groups according with the level of pain of arthrosis and muscle tissue. Eleven patients are en- rolled in the severe pain group and forty three are enrolled in the mild pain group, the items of common laboratory indexes of in- fectivity as the number of leukocyte （ WBC）, Erythrocyte Sedimentation Rate, C - reactive protein, Procalcitonin, and IgM, IgA, IgG, Complement 3 and Complement 4 are compared in the two groups. The difference of mean value of the length of hospi- tal stay and the interval time before relief are compared, respectively. Results The number of leukocyte, ESR, PCT and hu- moral immunity（IgM, IgG, IgA, C3 and CA）are not significant difference between the two groups（t = 1.94, 1.87, 1.64, 1.25, 1.16, 1.59, 1.47 and 1.61, P 〉 0. 05）. The mean value of CRP in severe pain group are （48. 5 ± 1.9） mg/L and （ 11.4 ±1.6） mg/L in mild pain group. The former is higher than the latter significantly（t =2. 24, P 〈0. 05）. The mean value of interval time before relief are （20. 5 ± 1.6） days and （ 12. 4 ± 1.7 ） days in the two groups, it is significant difference （ t = 2. 25, P 〈 0. 05）. The mean value of the length of hospital stay are（21.2± 3.7） days and（ 14. 6 ±3.3 ） days in the two groups, it is also significant difference（ t = 2. 37, P 〈 0. 05）.
