摘要
目的：通过对骨巨细胞瘤患者采用局部刮除肿瘤联合放疗的方法进行治疗，观察联合疗法对患者血清 IL-1、TNF -α、VEGF、M-CSF、HIF 及 NTx 水平的影响.方法：回顾性分析65例2012年1月~2016年2月在保定市第一中心医院进行诊治并接受住院综合治疗的长骨骨巨细胞瘤患者，分为对照组和观察组，两组患者一般资料之间无显著性差异.对照组28例患者，采用局部肿瘤刮除术进行治疗，观察组患者在此基础上配合放疗进行治疗，对两组患者治疗前后的血清 IL-1、TNF -α、VEGF、M-CSF、HIF 及 NTx 水平进行检测和比较.结果：（1）治疗前，两组患者血清 IL-1、TNF -α、VEGF 水平之间进行统计学比较，差异无显著性（P ＞0．05）；治疗后，两组患者血清 IL-1、TNF -α、VEGF 水平均明显下降，与同组治疗前相比，差异均具有显著性（P ＜0．05），且观察组患者血清 IL-1、TNF -α、VEGF 水平下降程度均明显低于同期对照组患者，差异具有显著性（P ＜0．05）.（2）治疗前，两组患者血清 M-CSF、HIF、NTX 水平之间进行统计学比较，差异无显著性（P ＞0．05）；治疗后，两组患者血清 M-CSF、HIF、NTX 水平均明显下降，与同组治疗前相比，差异均具有显著性（P ＜0．05），且观察组患者血清 M-CSF、HIF、NTX 水平下降程度均明显低于同期对照组患者，差异具有显著性（P ＜0．05）.结论：局部刮除肿瘤联合放疗可以明显改善骨巨细胞瘤患者血清 IL-1、TNF -α、VEGF、M-CSF、HIF、NTX 水平，而骨巨细胞瘤患者血清 IL-1、TNF -α、VEGF、M-CSF、HIF、NTX 水平异常变化可以为骨巨细胞瘤的诊断和病变程度的判断提供一定的参考.
Objective：To observe the effect of local curettage combined with radiotherapy on serum levels ofIL-1,TNF-α,VEGF,M-CSF,HIF and NTx in patients with giant cell tumor of bone.Methods：A total of 65 patients with giant cell tumor of long bone that accepted the diagnosis and comprehensive treatment in our hospital from January 2012 to February 201 6 were divided into control group and observation group,using the method of retrospective analysis.Another 28 patients in control group were treated by local tumor curettage,patients in the observation group were treated with radiotherapy on the ba-sis of control group,and the serum levels of IL-1,TNF-α,VEGF,M-CSF,HIF and NTx were detected and compared be-tween the two groups before and after treatment.Results：Before treatment,there were no significant differences in the serum levels of IL-1,TNF -α,VEGF between the two groups （P 〉0.05）;After treatment,serum TNF,IL-1,VEGF levels in the two groups of patients were significantly decreased compared with the same group before treatment,and the difference was sig-nificant （P 〈0.05）,the levels of serum TNF,IL-1,VEGF in the observation group were significantly lower than that in the control group （P 〈0.05）.Before treatment,there was no significant difference in the serum levels of M-CSF,HIF,NTX be-tween the two groups （P 〉0.05）;After treatment,serum M-CSF,HIF,NTX levels in the two groups of patients were signif-icantly decreased compared with the same group before treatment,and the difference was significant（P 〈0.05 ）,meanwhile, the levels of serum M-CSF,HIF,NTX in the observation group were significantly lower than that in the control group （P 〈0.05）.Conclusions：Local curettage combined with radiotherapy can significantly improve the serum levels of IL-1,TNF -α, VEGF,M-CSF,HIF,NTX in patients with giant cell tumor of bone,and the abnormal changes of serum levels of IL-1, TNF -α,VEGF,M-CSF,HIF,NTX can provide some reference for the diagnosis of giant cell tumor of bone and the degree o
