摘要
目的观察通心络胶囊联合普罗布考治疗缺血性卒中伴颈动脉斑块患者疗效及对血清氧化型低密度脂蛋白(ox-LDL)、基质金属蛋白酶-7(MMP-7)、白细胞介素-18(IL-18)及颈动脉内膜中层厚度(IMT)的影响。方法将106例缺血性卒中伴颈动脉斑块患者随机分为观察组53例和对照组53例,对照组给予普罗布考治疗,观察组给予普罗布考+通心络胶囊治疗,2组均连续治疗12个月;检测2组治疗前后血清ox-LDL、MMP-7、IL-18水平,记录2组治疗前后IMT、斑块面积、Crouse积分、斑块回声的变化情况。结果 2组治疗后血清ox-LDL、MMP-7、IL-18水平及IMT、斑块面积、Crouse积分均较治疗前显著降低(P均<0.05),且观察组上述指标均显著低于对照组(P均<0.05);2组治疗后低回声斑块比例均明显降低(P均<0.05),而高回声斑块比例均明显增高(P均<0.05),且观察组治疗后低回声斑块比例明显低于对照组(P<0.05),而高回声斑块比例明显高于对照组(P<0.05);2组治疗期间均未发现严重的药物相关不良事件。结论通心络胶囊联合普罗布考能够抑制缺血性卒中伴颈动脉斑块患者的血清ox-LDL、MMP-7、IL-18水平,对延缓或逆转颈动脉粥样硬化斑块进展具有一定的临床意义。
Objective It is to observe the curative effect of Tongxinluo capsule combined with Probucol on ischemic stroke with carotid plague and the effect on serum ox- LDL, MMP- 7, IL- 18 and carotid intima-media thickness （IMT）. Meth- ods 106 patients with ischemic stroke with carotid plague were randomly divided into observation group （ n = 53 ） and control group （n = 53） , the control group was treated with Probucol, the observation group was treated with Probucol ＋ Tongxinluo capsule. Both groups were treated for 12 months. The serum levels of ox - LDL, MMP - 7 and IL - 18 were determined, while IMT, plaque area, Crouse scores, plaque echo were detected or evaluated before and after treatment in both groups. Results The serum levels of ox- LDL, MMP- 7, IL- 18 and IMT, plaque area, Crouse scores were significantly decreased after treatment compared with that before treatment in both groups （ P 〈 0.05 ）, and the decreases in observation group were more significant than that in control group （ P 〈 0.05 ）. The proportion of low echo was lower while that of high echo was higher after treatment than that before treatment in both groups （P 〈 0.05）, the proportion of low echo was lower while that of high echo was higher in observation group than that in control group （ P 〈 0.05 ）. No severe side effect was found during treatment in both groups. Conclusion Tongxinluo capsule combined with Probucol could inhibit the levels of serum ox - LDL, MMP -7, IL- 18 in the patients with ischemic stroke with carotid plague, this effect has some clinical significance for slowing down or reversing the development of carotid atherosclerotic plague.
