运动疗法对人工膝关节置换术后下肢深静脉血栓形成预防的效果观察 被引量:24

摘要 目的:探讨运动疗法对人工膝关节置换术(TKA)患者术后下肢深静脉血栓(DVT)形成预防效果。方法:选取本院收治并行TKA治疗的骨关节炎患者153例,术后随机分为两组,对照组术后进行持续被动运动(CPM)训练,观察组术后进行CPM联合运动疗法治疗,治疗2周后检测并比较两组相关凝血指标、股静脉血流速度、DVT发生率。结果:两组术后2周~2个月凝血酶原时间(PT)、部分凝血酶原时间(APTT)与术前比较呈先缩短后延长趋势,纤维蛋白原(FIB)及D-二聚体(D-D)含量呈先升高后降低趋势(P〈0.05~0.01),两组术后2个月时凝血指标水平差异显著(P〈0.05);两组术后1周股静脉血流峰度及平均速度较术前升高(P〈0.05),观察组术后1周股静脉血流峰度及平均速度明显高于对照组(P〈0.05);观察DVT发生率明显低于对照组(P〈0.01)。结论:TKA术后患者行CPM训练联合运动疗法可有效预防患者DVT的形成,效果显著。 Objective: To explore the effect of exercise therapy in preventing the lower extremity deep venous thrombo- sis (DVT) after total knee arthoplasty (TKA). Methods: A total of 153 patients with osteoarthritis who were admitted in our hospital for TKA were included in the study and randomized into the observation group and the control group. The patients in the observation group were given continuous passive motion (CPM) in combined with exercise therapy, while the patients in the control group were only given CPM. After 2-week treatment, the related coagulation indicators and femoral venous blood flow in the two groups were detected and compared. The occurrence rate of DVT in the two groups was calculated. Results.. PT and APTT from 2 weeks to 2 months after operation in the two groups were shortened first and extended later when compared with before operation, while FIB and D-D contents were elevated first and reduced later (P〈0.05 or P〈0.01), and the coagu- lation indicator levels 2 months after treatment in the two groups were significantly different from those before operation (P〈 0.05). The femoral venous blood flow peak and average velocity 1 week after operation in the two groups were significantly ele- vated when compared with before operation (P〈0.05). The femoral venous blood flow peak and average velocity 1 week after operation in the observation group were significantly higher than those in the control group (P〈0.05). The occurrence rate of DVT in the observation group was significantly lower than that in the control group (P〈0.05). Oonelusions: CPM in combined with exercise therapy for patients after TKA can effectively prevent the formation of DVT, with a significant effect.
