摘要
目的利用SWE成像评估更年期妇女的颈动脉弹性模量、口服雌激素前后颈动脉弹性模量的变化以及雌二醇水平与颈动脉弹性模量的相关性。方法选取2014年1月至2016年5月在西安交通大学第一附属医院更年期门诊就诊的199例更年期妇女,包括激素治疗组97人,非激素治疗组102人,利用Supersonic SWE成像测得双侧颈动脉纵向平均弹性模量(Emean),并检测口服雌激素前后的雌二醇(E2)水平和双侧颈动脉前壁20个感兴趣区平均弹性模量的平均值(MEmean),对检测结果进行分析。结果 1小剂量口服雌激素后,激素治疗组颈动脉的僵硬度得到明显的改善,弹性模量较治疗前以及非激素治疗组明显减小(t值分别为-3.13、2.71,均P=0.001<0.01);2治疗后的颈动脉弹性模量较治疗前明显减小,差异具有统计学意义(t=3.24,P<0.05);3颈动脉弹性模量与雌激素水平呈负相关(r=0.79,P<0.05)。结论利用SWE成像可以对更年期妇女的颈动脉弹性进行量化,小剂量长期服用雌激素可以改善颈动脉的弹性,使动脉管壁僵硬度降低,更年期妇女的激素治疗对动脉硬化的预防起一定的作用。
Objective To evaluate the carotid artery elastic modulus of menopausal women,the changes of carotid elastic modulus before and after oral estrogen,and the relevance between estrogen and carotid artery elasticity by using SWE imaging. Methods A total of 199 menopausal women were selected from menopausal department of the First Affiliated Hospital of Xi＇an Jiaotong University from January2014 to May 2016. including 97 cases in hormone treatment group and 102 cases in non-treatment group. The longitudinal mean elastic modulus of bilateral carotid artery（ Emean） were measured using Supersonic SWE imaging,and the estrogen levels before and after oral estrogen were detected,and the average value of mean elastic modulus（ MEmean） of 20 interested areas at superficial walls of bilateral carotid artery were also detected. The results were analyzed. Results After oral administration of small dose estrogen,the stiffness of carotid artery in treatment group was improved obviously,and the elastic modulus reduced significantly compared to that before treatment and that in non-treatment group（ t value was- 3. 13 and 2. 71,respectively,both P = 0. 001 0. 01）. Elastic modulus reduced remarkably after treatment with statistical significance（ t = 3. 24,P〈0. 05）. The carotid artery elasticity was negatively correlated with estrogen level（ r = 0. 79,P〈0. 05）. Conclusion SWE imaging can be used to quantify the carotid artery elasticity of menopausal women.Long-term administration of small dose estrogen can improve the elasticity of carotid artery and reduce the carotid arterial stiffness. HRT of menopausal women plays a role in the prevention of arteriosclerosis.
