噻托溴铵联合无创通气治疗慢性阻塞性肺疾病合并呼吸衰竭的效果观察 被引量:7

Efficacy of tiotropium combined with noninvasive ventilation in the treatment of chronic obstructive pulmonary disease complicated with respiratory failure

摘要 目的探讨噻托溴铵(能倍乐)与无创通气(NPPV)联用对慢性阻塞性肺疾病(COPD)合并呼吸衰竭(RF)患者的价值。方法选取2013年10月至2015年10月我院住院治疗的COPD合并RF患者60例,随机分为2组,每组30例。对照组接受基础治疗加NPPV,观察组在对照组的基础上给予能倍乐,疗程均为12周。治疗前后观察患者的肺功能指标:1s用力呼气容积(FEV1)、用力肺活量(FVC)和FEV1/FVC;血气分析指标:动脉血氧分压(PaO_2)、动脉二氧化碳分压(PaCO_2)及pH值。结果治疗后,两组的FEV1、FVC、FEV1/FVC和PaO_2指标比治疗前明显升高(P<0.05),PaCO_2则明显下降(P<0.05),且以观察组改善更显著(P<0.05)。结论能倍乐联合NPPV治疗能改善COPD合并RF患者的肺功能和动脉血气,有助于患者康复。 Objective To explore the significance of a combination of tiotropium bromide and noninvasive positive pressure ventilation (NPPV) in the treatment of chronic obstructive pulmonary disease (COPD) complicated with type II respiratory failure (RF). Methods A total of 60 patients with COPD complicated with RF receiving treatment in our hospital from October 2013 to October 2015 were selected and randomly divided into two groups, with 30 cases in each group. Control Group received basic treatment combined with NPPV while Observation Group received treatment with tiotropium bromide based on the treatment for Control Group, and the disease course is 12 weeks. The pulmonary function indicators of the patients were observed before and after treatment: the first second forced expiratory volume(FEV1), forced vital capacity (FVC) and FEV1/FVC; blood gas analysis indicators: PaO2, PaCO2 and pH. Results After the treatment, the level of FEV1, FVC, FEV1/FVC and PaO2 increased significantly (P〈0.05) while the level of PaCO2 decreased significantly (P〈0.05)compared with those before treatment, and Observation Group showed a more significant improvement (P〈0.05). Conclusion The combination oftiotropium bromide can obviously improve lung function and arterial blood gas of the patients with COPD complicated with RF, which is worth clinical application.
