摘要
目的：自体 CIK 细胞联合培美曲塞和顺铂治疗非小细胞肺癌的临床效果研究。方法选取2012年2月至2015年7月于我院诊治的非小细胞肺癌患者共87例进行研究,按照随机原则分为研究组（n =44）和对照组（n =43）。对照组实施培美曲塞和顺铂一线治疗,研究组采用自体 CIK细胞联合培美曲塞和顺铂治疗。比较2组患者治疗前后免疫功能变化情况,评价临床疗效、生活质量评分、药物毒性反应。结果研究组患者在治疗后的各项免疫功能指标（自然杀伤细胞、CD3＋、CD4＋、CD8＋、CD4＋/CD8＋）均明显优于治疗前、对照组（P 值均〈0.05）；研究组和对照组的疾病控制率分别为86.36%和65.12%,研究组均高于对照组（χ2=5.363,P 〈0.05）；研究组生活质量总稳定率（52.27%）高于对照组（18.60%）（χ2=10.752,P 〈0.05）；研究组患者的胃肠道反应发生率（Ⅰ-Ⅱ度）、中性粒细胞减少发生率（Ⅰ-Ⅱ度、Ⅲ-Ⅳ度）均低于对照组（χ2值分别为12.164、11.553、5.132,P 值均〈0.05）。结论对非小细胞肺癌患者行自体 CIK 细胞联合培美曲塞和顺铂治疗,患者免疫功能、生活质量明显改善,疾病控制率高,药物毒性反应低,效果显著,值得推广。
Objective To evaluate the clinical effect of autologous CIK cells combined with pemetrexed and cisplatin in treatment of non-small cell lung cancer.Methods 87 cases of non-small cell lung cancer in our hospital from February 2012 to July 201 5 were chosen.And all patients were divided into the study group （44 cases） and the control group （43 cases） according to the random principle.The control group was treated with the pemetrexed and cisplatin,while the study group was treated with autologous CIK cells,pemetrexed and cisplatin.The changes of immune function before and after treatment were compared,the clinical curative effect,quality of life score,drug toxicity reaction in two groups were evaluated.Results After treatment,the immune function indexes （natural killer cell,CD3 ＋ , CD4 ＋ ,CD8 ＋ ,CD4 ＋/CD8 ＋） in the study group were significantly better than those before treatment and the control group （all P 〈0.05）.The disease control rates of the study group and the control group were 86.36% and 65.12% （χ2 =5.363,P 〈0.05）,and the total quality of life in the study group （52.27%） was higher than that in the control group （18.60%）（χ2 = 10.752,P 〈 0.05）.The incidences of gastrointestinal reaction（Ⅰ-Ⅱ degree）,neutrophil reduction （Ⅰ-Ⅱ degree,Ⅲ-Ⅳ degree） in the study group were lower than those in the control group（χ2 =12.1 64,1 1.553,5.132,all P 〈0.05）.Conclusions Autologous CIK cells combined with pemetrexed and cisplatin are used in treating the patients with non-small cell lung cancer,the immune function and quality of life are significantly improved,the disease control rate is high,the drug toxicity reaction is low,the effect is remarkable,and it is worth popularizing.
