摘要
目的探讨急性胰腺炎患者胰腺CT表现与血浆D-二聚体水平的相关性。方法选取2014年1月至2015年10月于本院接受治疗的28例急性胰腺炎患者为研究对象。所有患者均于入院即刻及入院后第3天和第7天清晨检测血浆D-二聚体水平,同时行腹部胰腺CT,并于第7天行腹部增强胰腺CT平扫检查。比较入院即刻及入院后第3天、第7天不同疾病程度和不同疾病分型急性胰腺炎患者血浆D-二聚体水平变化情况。结果不同时间点重症急性胰腺炎患者血浆D-二聚体水平均高于轻症急性胰腺炎患者(P<0.05)。入院7天内,重症急性胰腺炎患者血浆D-二聚体水平逐渐升高;轻症急性胰腺炎患者血浆D-二聚体水平在入院即刻和入院后第3天呈上升趋势,但入院后第7天略有下降。不同时间点,坏死性胰腺炎患者血浆D-二聚体水平均显著高于水肿性胰腺炎患者(P<0.05)。入院7天内,坏死性胰腺炎患者血浆D-二聚体水平逐渐升高,而水肿性胰腺炎患者血浆D-二聚体水平在入院即刻和入院后3天均逐渐升高,但入院3天后转而下降。结论急性胰腺炎患者胰腺CT表现与血浆D-二聚体水平具有相关性,胰腺CT检查联合血浆D-二聚体水平检测能够在一定程度上辅助判断疾病进程。
Objective To explore the relationship between pancreatic CT manifestations and plasma D-dimer level in patients with acute pancueatitis. Method 28 cases of patients with acute pancreatitis in our hospital from January 2014 to October 2015 were selected as the research objects. Plasma D-dimer level and abdominal CT manifestations were checked in the early morning on admission, the 1st, 3rd and 7th day. Abdominal enhanced pancreatic CT scan was performed on the 7th day. The changes of plasma D-dimer level in patients with acute pancueatitis of different classification and degree were compared on admission, the 3rd and 7th day. Result Plasma D-dimer levels at different time points in severe acute pancreatitis patients were higher than mild acute pancreatitis patients (P 〈 0.05), the plasma D-dimer levels in patients with severe acute pancreatitis increased within 7 days, and in patients with mild acute pancreatitis, plasma D-dimer level was on the rise on admission and the 3rd day, but in a slight decline on the 7th day. Plasma D-dimer levels in patients with necrotizing pancreatitis were significantly higher than in patients with edematous pancreatitis at different time points (P 〈 0.05). Plasma D-dimer level increased in patients with necrotizing pancreatitis within 7 days, and in patients with edematous pancreatitis, plasma D-dimer level increased on admission and 3rd day, but turned down after 3 days. Conclusion The correlation between pancreatic CT manifestations and plasma D-dimer level is assured, pancreatic CT examination combined with D-dimer testing can determine the disease process in a certain extent auxiliarily.
