摘要
目的：观察人工真皮（皮耐克）联合封闭引流在瘢痕修复中的应用效果。方法回顾性分析2014年8月至2015年9月瘢痕挛缩患者20例，Ⅰ期行瘢痕切除和松解，若关节部位保持关节完全伸直位，人工真皮移植，封闭负压引流（VAC）。2周后II期手术：去除VAC及皮耐克硅胶膜行自体刃厚皮移植及再次VAC。结果20例患者皮片均存活，3例失访，2例皮下少许积血渗出，经换药后愈合，愈合后再次瘢痕挛缩，但较术前好转，余患者术后瘢痕无挛缩，外观满意，质地柔软有弹性，颜色接近周围正常皮肤，功能恢复可。结论人工真皮及刃厚皮片联合封闭负压引流在瘢痕整形中有良好效果。
Objective To research the application of artifical dermis（PELNAC）combined with negative pressure drainage in repairing scar. Methods To analyse 20 patients with scar contracture during August in 2014 to September in 2015. In the first stage,all patients underwent scar excision and release. If the scar located in the joints,and keep straight joints completely. Following PELNAC grafting. All patients were managed with VAC dressing. Two weeks later,in the second stage,after removing VAC dressing and skin Nike silicone membrane,and then accepted autologous split-thickness skin grafting,and were managed with VAC dressing. Results All flaps in the 20 patients survived,and three patients were lost to follow-up,two patients were healed after changing their dressing who had subculaneous hemorrhage and effusion,then the two cases occur scar contracture again,but better than before. Others postoperative scars were contracture free andthe appearance were of satisfaction with the texture being soft and flexible,color being similar to the surrounding normal skin,and good functional recovery. Conclusion Artifical dermis（PELNAC）and split-thickness skin combined with negative pressure drainage in scar plastic surgery have desirable effect.
