应用矫形外固定器治疗近指间关节屈曲挛缩 被引量:7

Application of orthopedic external fixation in treatment of proximal interphalangeal joint fiexion contracture

摘要 目的分析应用矫形外固定器治疗手指近指间关节屈曲挛缩并探讨其疗效。方法2011年8月至2015年12月,广东医学院附属高明医院手外科对手部外伤导致近指间关节屈曲挛缩28例(36指)应用矫形外固定器手术治疗,术后通过1~2个月调整外固定器牵引矫形使关节囊韧带及皮肤瘢痕延长达到挛缩松解、改善关节活动度。手功能恢复按中华医学会手外科学会发布的手部肌腱修复后评定标准(TAM)评价。结果28病例(36指)无钉孔感染,伤指关节屈曲挛缩矫正,屈伸功能良好。术后随访6~37个月(平均21个月),TAM评价结果为优16例,良12例。结论采用矫形型外固定器治疗合并皮肤瘢痕挛缩的手指近指间关节屈曲挛缩,关节挛缩矫形效果明显,手指关节活动度增加且稳定,疗效满意。 Objective To analyze the effects of orthopaedic type external fixator on treatment of finger joint flexion contracture and discuss the curative effect. Methods From August 2011 to December 2015, in the hand surgery department of the affiliated Gaoming Hospital of Guangdong Medical College, 28 cases (36 fingers) of traumatic flexion eontracture of the interphalangeal joint accepted orthopaedic surgery by external fixator. After one to two months adjustment of external fixator traction, the joint capsule, ligaments and skin scar were all extended and the eontractures were released; the range of motion was improved. The hand function restoration was assessed by total active movement (TAM) which is the criteria for assessment of hand tendon repair released by Hand Surgery Society of Chinese Medical Association. Results The flexion contractures of the injured knuckles in 28 eases (36 finger) were corrected without nail infection; the functions of flexion and extension were good. The postoperative follow-up last for 6 - 37 months (average, 21 months). The hand function restoration assessed by TAM showed that the results of 16 cases were excellent and 12 cases were good. Conclusion The external fixator may well treat the finger flexion contracture combining the skin scar contracture of the interphalangeal joint; corrective effect of the joint contracture is obvious, the range of motion in finger joint may increase leading to satisfying curative effects.
