摘要
目的:分析慢性心力衰竭患者血清半乳糖凝集素3(galectin-3,Gal-3)与心功能分级、心脏左室射血分数(LVEF)及B型利钠肽(B-type natriuretic peptide,BNP)的相关性,探讨Gal-3在慢性心力衰竭诊断中的价值。方法:选取慢性心力衰竭患者200例和健康体检者200例,并将慢性心力衰竭患者根据心衰程度分为NYHA心功能Ⅱ级、Ⅲ级、Ⅳ级,检测各组三酰甘油、总胆固醇、血尿素氮、血肌酐、糖化血红蛋白(Hb A1c)、BNP、Gal-3以及心脏彩超测定LVEF。结果:慢性心衰组血清Gal-3水平[(34.88±11.86)μg/L]明显高于健康对照组[(15.67±6.20)μg/L,P<0.01]。在慢性心衰组,随着NYHA心功能分级升高,血清Gal-3水平也显著升高(P<0.01)。相关性分析显示,Gal-3与BNP(r=0.504,P<0.01)、NYHA心功能分级(r=0.756,P<0.01)正相关,与LVEF(r=-0.678,P<0.01)负相关。ROC曲线显示血清Gal-3水平对慢性心力衰竭具有诊断价值(AUC=0.792,P<0.01),诊断慢性心力衰竭的最佳Gal-3水平为18μg/L。结论:心衰患者血清Gal-3水平明显升高,且与心功能分级相关,可作为慢性心力衰竭的临床诊断指标。
Objective：To analyze the correlation between serum galectin-3 （Gal-3）with heart function, left ventricular ejection fraction （LVEF）and B-type natriuretic peptide （BNP）in chronic heart failure （CHF）,and evaluate the value of Gal-3 in diagnosis of CHF.Methods：A total of 200 patients with CHF were divided into three groups according to their heart function：NYHA class Ⅱ,class Ⅲ and class Ⅳ;and 200 normal subjects enrolled as control group.Clinic parameters,including triacylglycerol,total cho-lesterol,blood urea nitrogen （BUN ）,serum creatinine （SCr）,glycosylated hemoglobin （HbA1 c ）,BNP and Gal-3 were determined and echocardiography was tested for each participant.Results：Compared with control group,Gal-3 levels in CHF group were significantly elevated [（34.88 ±1 1 .86）μg/L vs.（1 5.67 ± 6.20）μg/L,P〈0.01 ].With the deterioration of NYHA heart function in CHF group,concentration of serum Gal-3 increased gradually（P〈0.01 ）.The correlation analysis showed that the serum Gal-3 concen-tration was positively related to BNP （r=0.504,P〈0.01 ）,NYHA heart function （r=0.756,P〈0.01 ） and negatively related to LVEF （r =-0.678,P 〈0.01 ）.The receiver-operating characteristic curve （ROC）analysis showed that Gal-3 could be a predictor of CHF with an area under the curve （AUC）0.792 （P〈0.01 ）and the cutoff point for Gal-3 was 1 8 μg/L.Conclusion：The serum Gal-3 concentration in CHF group was significantly elevated,positively related to NYHA heart function and could be used as diag-nostic criteria of CHF.
