摘要
目的：探讨质量管理工具在我院门诊药房调剂差错中的应用价值。方法应用PDCA循环及质量管理工具如调查表、甘特图、鱼骨图等，分析我院2015年上半年（1～6月）药房调剂差错事件原因，探索对策、予以实施并进行持续改进。比较方案实施前后药房调剂差错事件变化。结果与方案实施前相比，2015年下半年（7～12月）药房调剂差错事件由35件／月下降到活动后的17件／月，不仅处方调剂差错数量减少，药学人员的团队合作精神以及应用质量管理工具的能力也有较大提升。结论运用PDCA循环和质量管理工具，能够不断发现和解决问题，明显减少我院药房调剂差错事件，使药房调剂工作得以持续改进。
Objective To explore the applying value of the quality management tools in dispensing error in our hospital. Methods By means of PDCA cycle and the quality management tools,including investigative form,gantt diagram,fishbone diagram, etc.to analyze the conditions of dispensing error of our hospital in the first half of 2015,and to look for the reasons for dispensing error, explore countermeasures,implement and continuous improvement.Results According to the continuous improvement,compared with Jan.to Jun.2015 35 times/month,the dispensing error was dropped to 17 times/month,reducing the number of prescription dispensing errors,and increasing teamwork and the application of quality management tools.Conclusion Application of PDCA cycle and quality management tools could significantly reduce the dispensing errors,discover and solve problems,make the pharmacy dispenging work continuously improved.
