摘要
目的探讨载脂蛋白C3(apolipoproteinC3,APOC3)对急性缺血性脑卒中(acute isehemic stroke,AICS)严重程度的评价及预测价值。方法选择2015年3月-2015年12月在徐州医科大学附属医院住院的210例AICS患者作为研究对象(AICS组),按美国国立卫生研究所卒中量表(National Institutes of Health Stroke,NIHSS)评分进行神经功能缺损程度评估,将210例AICS患者分为轻度组(〈4分,57例)、中度组(4—15分,139例)及重度组(〉15分,14例)3个亚组;选择同期来我院的健康体检者90例作为对照组。采集两组静脉血标本检测血脂、空腹血糖,应用酶联免疫吸附法(Enzyme linked immunosorbent assay,ELISA)测定血浆APOC3浓度,利用相关与ROC曲线等统计学方法分析APOC3对AICS严重程度的评价及预测价值。结果AICS组空腹血糖、甘油三酯、低密度脂蛋白、载脂蛋白B、脂蛋白(a)含量明显高于对照组(P〈0.05),而高密度脂蛋白含量低于对照组(P〈0.05)。AICS组血浆APOC3含量明显高于对照组,且重度组〉中度组〉轻度组,差异均有统计学意义(P〈0.05);Pearson相关性显示APOC3与NHISS相关密切(Pearson系数0.854,P〈0.05);发生缺血性脑卒中ROC曲线下面积为0.804。结论AICS患者呈现APOC3高表达状况,与疾病严重程度成正相关,且特异性及灵敏性较高,可能成为预测AICS的新型指标。
Objective To investigate the evaluation and predictive value of apolipoprotein C3 (APOC3) on acute ischemic cerebral stroke (AICS). Methods 210 patients with AICS in the Affiliated Hospital of Xuzhou Medical University from March 2015 to December 2015 were performed as AICS group, separating them into three different sub-groups according to National Institutes of Health Stroke Scale (NIHSS), patients were divided into mild group( 〈 4 score, 57cases), moderate group(4-15 score, 139cases) and severe group( 〉 15 score, 14cases). 90 cases were selected at the same period as control group. Fasting venous blood were taken from the above groups. Both groups were monitored the levels of blood lipid, fasting blood glucose , enzyme-linked immunosorbent assay (ELISA) was performed to detect the two groups of serum APOC3 concentration, the evaluation and predictive value of apolipoprotein C3 on acute ischemic stroke severity by relevant statistical methods and ROC cure analysis. Results The serum fasting plasma glucose, triglyceride, low-density lipoproteiu, apolipoprotein B and lipoprotein (a) levels of the AICS patients group were all signifiantly higher than those of the control group (all P〈 0.05); and the high-density lipoprotein level of the AICS group was significantly lower than that of the control group (P〈 0.05). The serum APOC3 level of the severe AICS group was significantly higher than that of the moderate AICS group, and the serum APOC3 level of the moderate AICS group was significantly higher than that of the mild AICS group, and the more severe the condition of AICS the higher the serum APOC3 level (all P 〈0.05). In the AICS group, the serum APOC3 level was closely correlated with the level of NIHSS (Pearson coefficient of 0.854, P 〈0.05). Receiver operating characteristic curve (ROC curve) showed that the area under the curve was 0.804. Conclusion The serum APOC3 level is positively correlated with the severity of condition of AICS.
