摘要
目的探讨妊娠期糖尿病(GDM)孕妇在不同孕期及产后血清Chemerin、RBP4指标的表达及意义。方法采用酶联免疫吸附法测定165例GDM患者(GDM组)及44例糖耐量正常孕妇(对照组)孕中期及孕晚期的血清Chemerin、RBP4、空腹胰岛素(FINS)数值,并另加选108例GDM患者于产后42天检测以上指标。结果 GDM组孕妇在孕中期及孕晚期时的FINS、chemerin及RBP4均明显高于对照组,差异均有统计学意义(P<0.05);GDM孕妇的FINS、chemerin、RBP4数值均明显在孕晚期>孕中期>产后组,差异有统计学意义(P<0.05);产后GDM组的FINS、chemerin、RBP4数值均大于正常孕妇组孕中期时,差异有统计学意义(P<0.05)。结论 Chemerin、RBP4与GDM发病密切相关,参与了GDM孕妇的胰岛素抵抗,随孕周增加抵抗更明显。Chemerin、RBP4可能反映出GDM孕妇胰岛素功能受损状态,产后仍短期内仍难以恢复,其值对将来是否易患2型糖尿病可能有预测指标的作用。
Objective To investigate the expression and significance of Chemerin and RBP4 in serum of pregnant women with gestational diabetes mellitus( GDM) at different stages of pregnancy and postpartum.Methods The serum Chemerin and RBP4 levels in 165 cases of patients with GDM( GDM group) and in 44 cases of pregnant women with normal glucose tolerance( control group) of the second trimester and third trimester were determined individually by Enzyme-linked immunosorbent assay( ELISA),and the same indicators were monitored with another 108 cases of GDM patients in the 42 days after delivery. Results( 1) The serum level of FINS,chemerin and RBP4 were significantly higher in GDM group than that in control group of the second trimester and third trimester( P〈0.05).( 2) The serum level of FINS,Chemerin and RBP4 with GDM in the third trimester were higher than the second trimester and higher than postpartum( P〈0. 05).( 3) The serum level of FINS,Chemerin and RBP4 in GDM group were higher than those in normal pregnant women during the second trimester of pregnancy( P〈0.05). Conclusions Chemerin and RBP4 were closely related to the insulin resistance in GDM pregnant women. This phenomenon was more obvious with the increase of the gestational age resistance.Chemerin and RBP4 may reflect the impaired insulin function in GDM pregnant women,postpartum is still difficult to recover in the short term,it may be possible to predict the future of GDM in pregnant women with type2 diabetes.
