摘要
目的探讨在超声引导下运用麦默通（Mammotome）微创旋切系统行皮下乳腺切除治疗男性乳腺发育症的临床运用价值。方法2013年6月至2015年4月宿迁市人民医院乳腺外科在超声引导下运用麦默通微创旋切系统对23例男性乳腺发育症患者施行了皮下乳腺切除术，回顾性分析临床资料，对患者术后的切口瘢痕、胸壁外形及乳头乳晕等方面进行综合评价。结果23例手术均获成功，未出现术中并发症。手术时间30～65min，平均42min；术后未发生切口感染、裂开或愈合不佳；术后住院3—5d。随访6—15个月，患者手术部位瘢痕小且隐蔽，乳头乳晕处无明显凹陷，外形美观，乳头感觉正常，所有患者对手术效果表示较满意。结论在超声引导下运用麦默通微创旋切系统治疗男性乳腺发育症，手术操作简单、安全，术后胸部外观及功能等方面效果较好，瘢痕隐蔽不明显，患者满意率高。
Objective To investigate the clinical application value of ultrasound-guided Mammotome minimally invasive rotatory cutting System applying on the subcutaneous mammary gland resection for gynecomastia. Methods We retrospectively analyzed the clinical data of 23 gynecomastia patients, from June 2013 to April 2015 in the department of breast surgery of Suqian People＇s Hospital, who were treated by subcutaneous mammary gland resection using uhrasound-guided Mammotome minimally invasive rotatory cutting system. The postoperative incision scar, shape of thorax, nipple and mammary areola were evaluated comprehensively. Results All the 23 surgeries were successful and no complications happened during the surgery. The operating time lasted about 30 to 65 minutes and the average operating time was 42 minutes. No postoperative incision infection, wound dehiscence or poor healing happened on the patients. The postoperative hospital stay was about 3 to 5 days. The patients were followed up for 6 to 15 months. The scar of the patients＇ surgical sites were small and hidden. No significant depression of the nipple and areola and nipple paresthesia happened. All the patients were satisfied with the surgery outcome. Conclusions The ultrasound-guided Mammotome minimally invasive rotatory cutting system is easy-operating and safe on the treatment of gynecomastia. The recovery of the shape and function of the chest after surgery is better than regular one, especially the incision scar is small and hidden and patients＇ rate of satisfaction is high.
