摘要
目的:采用超声技术检测部队体检者颈动脉内-中膜(intima-media thickness,IMT)的厚度,分析颈动脉IMT增厚患者的临床特点及影响因素。方法:选取2011年6月至2015年6月武警后勤学院附属医院干部病房体检人员共1 578例,分别进行病史采集、实验室检查及颈动脉超声等检查,按照颈动脉IMT是否增厚分为2组,比较2组间临床指标,分析颈动脉IMT增厚的影响因素。结果:IMT增厚组年龄、体质量指数、收缩压、甘油三酯、低密度脂蛋白、空腹血糖、血浆同型半胱氨酸均高于非增厚组,差异有统计学意义(P<0.01),且多元逐步回归分析显示年龄、收缩压、空腹血糖、同型半胱氨酸、甘油三酯、低密度脂蛋白是IMT增厚的独立危险因素(优势比OR值分别为0.908、2.313、1.300、1.625、1.249、2.191)。结论:早期用超声检测颈动脉,及早控制可能影响颈动脉IMT增厚的危险因素,对预防和治疗动脉硬化性疾病具有指导作用。
Objective To execute ultrasonography of the carotid artery intima-media thickness(IMT) of military physical examinees and to analyze the clinical characteristics and influencing factors of thickened IMT patients. Methods Totally 1 578 examinees in the cadre ward of the Affiliated Hospital of Logistics University of PAP from June 2011 to June 2015 underwent history taking, laboratory examination and carotid artery ultrasonography, who were divided into two groups according to if the examinees had thickened IMT. The two groups were compared for clinical indexes, and the influencing factors were analyzed for thickened IMT. Results The group with thickened IMT had the indexes significantly higher than the other one such as age,body mass index, contractive pressure, triglyceride, low density lipoprotein, blood-fasting sugar and plasma homocysteic acid(P〈0.01). Multiple regression analysis proved that the independent risk factors for the thickened IMT included age, contractive pressure, blood-fasting sugar, homocysteic acid, triglyceride and low density lipoprotein, with the OR values were 0.908, 2.313,1.300, 1.625, 1.249 and 2.191, respectively. Conclusion The involvement of carotid artery ultrasonography and the risk factors controlling for thickened IMT in early stage contribute to the prevention and treatment of arteriosclerotic diseases.
