摘要
目的观察自拟健胃愈疡汤联合三联疗法对幽门螺杆菌(H.pylori)阳性十二指肠球部溃疡患者血浆胃肠激素、胃电图及生活质量的影响。方法将90例H.pylori阳性十二指肠球部溃疡患者随机分为观察组及对照组各45例,对照组给予甲硝唑片、阿莫西林胶囊、奥美拉唑肠溶胶囊三联疗法治疗,观察组在对照组治疗基础上加用自拟健胃愈疡汤治疗,2组均连续治疗8周。观察2组治疗后疗效及治疗前后血浆胃肠激素、胃电图及生活质量变化情况。结果观察组治疗后H.pylori根除率、胃镜疗效、临床症状体征疗效均明显优于对照组(P均<0.05);2组治疗后血浆胃泌素、肾上腺髓质素、胃动素、降钙素基因相关肽、生长抑素水平及胃电图相关指标胃肠电节律紊乱、平均频率、反应面积、平均幅值均较治疗前显著改善(P均<0.05),且观察组上述指标改善情况均明显优于对照组(P均<0.05);2组治疗后日常活动、精神面貌、睡眠质量、食欲及自身疾病认知评分均明显改善(P均<0.05),且观察组上述指标改善情况均明显优于对照组(P均<0.05);2组治疗期间均未出现不良反应。结论自拟健胃愈疡汤联合三联疗法能够有效调节H.pylori阳性十二指肠球部溃疡患者的血浆胃肠激素水平,改善胃电图相关指标及临床症状,提高生活质量,值得临床推荐。
Objective It is to observe the effect of Jianwei Yuyang decoction combined with triple thrapy on gastrointestinal hormones,electrogastrogram and life quality in patients with H. pylori positive duodenal bulb ulcer. Methods 90 patients with H. pylori positive duodenal bulb ulcer were randomly divided into observation group and control group,45 cases in each group.The control group was given triple therapy with metronidazole tablets,amoxicillin capsule and omeprazole enteric capsules,while the observation group was added with Jianwei Yuyang decoction. Both groups were treated for 8 weeks. The changes of gastrointestinal hormones,electrogastrogram and life quality before and after treatment in both groups were observed. Results The clearing rate of H. pylori,curative effect of gastroscopy,effect of clinical syndrome and sign in observation group were better than that in control group（ P〈0. 05）. The levels of plasma gastrin,adrenal medullin,motilin,calcitonin gene related peptide and somatostatin and the related parameters of electrogastrogram gastric electric rhythm disorders like average frequency,reaction area,average amplitude were significantly improved in both groups after treatment（ P〈0. 05）,and the improvements in observation group were better than that in control group（ P〈0. 05）. The scores of daily activities,mental outlook,sleep quality,appetite,and their own understanding disease were significantly improved in both groups after treatment（ P〈0. 05）,and the improvements in observation group were better than that in control group（ P〈0. 05）. No side effect was found during treatment in both groups. Conclusion Jianwei Yuyang decoction combined with triple therapy could effectively regulate plasma level of gastrointestinal hormones,improve correlated indexes of electrogastrogram and life quality in patients with H. pylori positive duodenal bulb ulcer,which is worthy of recommending in clinic.
