摘要
目的探讨急性阑尾炎MSCT检查阑尾周围改变对指导阑尾炎病理分型的临床价值。方法抽取2012年3月至2015年4月我院收治的100例急性阑尾炎患者作为研究对象,其中急性单纯性阑尾炎33例、急性化脓性阑尾炎33例、急性坏疽性阑尾炎患者34例,由1名未知结果的高年资CT诊断医师采用MSCT对患者阑尾周围进行扫查,分析MSCT检查分型结果的准确率及MSCT检查征象。结果经MSCT检查,急性单纯性阑尾炎、急性化脓性阑尾炎、急性坏疽性阑尾炎的诊断准确率分别为93.94%、90.91%及94.12%;急性单纯性阑尾炎、急性化脓性阑尾炎、急性坏疽性阑尾炎在脂肪厚度、管腔内积液、淋巴结肿大、网膜包裹、腔管外积气及直径方面比较,均具有统计学差异(P<0.05)。结论采用MSCT对阑尾周围变化情况进行检查,可明显提高急性阑尾炎患者临床病理分型的准确率,具有一定的临床价值。
Objective To explore the clinical value of MSCT to find peri-appendiceal changes for the pathological classification of appendicitis. Methods A total of 100 cases of appendicitis admitted to our hospital from March 2012 to April 2015 were selected as the research objects, including 33 cases of acute simple appendicitis, 33 cases of acute suppurative appendicitis and 34 cases of acute gangrenous appendicitis. All cases were given MSCT scan around the appendix by one senior CT radiologist who did not know the results. The accuracy of MSCT for pathological classification and MSCT scanning findings were analyzed. Results The diagnostic accuracy of MSCT for acute simple appendicitis, acute suppurative appendicitis and acute gangrenous appendicitis were 93.94%, 90.91% and 94.12% respectively. Among acute simple appendicitis, acute suppurative appendicitis and acute gangrenous appendicitis, the MSCT scanning findings including fat thickness, intraluminal fluid, lymph node enlargement, omental wrapping, gas accumulation outer cavity tube and diameter were statistically different （P〈0.05）. Conclusions MSCT used to inspect the peri-appendiceal changes can effectively improve the accuracy of pathological classification of appendicitis, which has high clinical value.
