不同浓度罗哌卡因腹横肌平面阻滞在腹腔镜下胆囊切除术后镇痛中的应用 被引量:10

摘要 目的:评价不同浓度罗哌卡因腹横肌平面阻滞在腹腔镜下胆囊切除术后的镇痛效果。方法:择期腹腔镜胆囊切除术80例患者,根据罗哌卡因浓度随机分为0.250%组、0.375%组、0.5%组和对照组4组,每组20例,均采用气管插管静吸复合全身麻醉。手术结束后超声引导两点法分别注射0.250%、0.375%及0.500%的罗哌卡因20m L行腹横肌平面(TAP)阻滞,合计共40m L,C组无TAP阻滞,各组拔除气管导管后行静脉自控镇痛(PCIA)镇痛,记录拔管后1h(T1)、术后6h(T2)、24h(T3)、48h(T4)的静息VAS疼痛评分,以及PCIA用量。结果:与C组比较,罗哌卡因0.250%、0.375%、0.500%组T1-T4时VAS评分静息时均明显降低(P<0.05);PCIA泵使用量均明显减少(P<0.05)。与罗哌卡因0.250%组比较,罗哌卡因0.375%、0.500%组PCIA泵使用量明显减少;T4时罗哌卡因0.500%组PCIA泵使用量(102.8±0.5 m L)明显少于其他3组(P<0.05)。结论:罗哌卡因腹横肌平面阻滞在术后48h内为腹腔镜下胆囊切除术患者提供良好的镇痛效果,浓度0.500%罗哌卡因效果最佳。
