摘要
目的:探讨白光支气管镜(WLB)联合荧光支气管镜(AFI)技术对支气管腔内肺癌早期的诊断价值。方法2014年1月1日~2016年5月30日,我院在WLB下发现支气管腔内黏膜病变疑为肺癌的患者91例,91例患者均进行AFI检查,并同时用活检钳进行活检做病理学诊断。结果91例患者均获得明确病理学诊断结果。91例患者有64例AFI荧光阳性,在64例AFI荧光阳性患者中病理学诊断肺癌56例,良性病变8例,27例AFI阴性患者中病理学诊断肺癌4例,良性病变23例。WLB联合AFI诊断支气管腔内肺癌的敏感性为93.3%,特异性为74.2%,诊断准确率达86.8%,阳性预测值87.5%,阴性预测值85.2%。结论 WLB联合AFI技术可以提高支气管腔内肺癌的诊断准确性,在支气管腔内肺癌的早期诊断中具有很高的应用价值。
Objective To evaluate the accuracy of combination of white light bronchoscopy(WLB)with autofluorescence imaging videobronchoscopy(AFI) in the diagnosis of endobronchial lung cancer. MethodsWe did AFI videobronchoscopy in 91 patients who weresuspected of endobronchial lung caner by WLB from 1st Jan 2014 to 30th May 2016. All the patients were biopsied with forceps under the guidance of AFI. The pathological examination was conducted by the pathologists of the same hospital.Results All 91 cases were deifnitely diagnosed with pathological results. 64 patients that were positive for autofluorescence, 56 were diagnosed as lung cancer and 8 were benign diseases. 27 patients that were negative for autolfuorescence, 23 were diagnosed as benign diseases and only 4 patients was diagnosed as lung cancer. The sensitivity and speciifcity of combination of WLB with AFI for endobronchial cancer was 93.3% and 74.2% respectively, and the diagnostic accuracy was 86.8%. The positive and negative predictive value was 87.5% and 85.2%.ConclusionThe results showed that combination of WLB and AFI is helpful in increasing the diagnostic accuracy of endobronchial lung cancer. In conclusion, AFI has critical role in the diagnosis of lung cancer.
