113例冠脉造影和介入治疗被检者所受辐射剂量统计与分析 被引量:6

Statistics and analysis of the radiation dose taken by coronary angiography(CAG) and percutaneous coronary intervention(PCI) examination among 113 cases

摘要 目的 通过对冠状动脉造影和介入治疗被检者所受辐射剂量值的统计、分析与评估,探讨如何降低其放射诊疗的风险度,避免确定性效应的发生.方法 收集113例受检者检查时在线记录全部辐射剂量值及其相关的参数值,进行分类统计,对照国家相关文件要求比对、分析与评估.结果 ①单纯冠脉造影(CA)69例,总累积剂量面积乘积(DAP)均值为(7257.8±7935.6)μGy/m2;总累积皮肤入射剂量(ESD)均值为(865.5±418.87)mGy,其中有6例>2 Gy,占9%;透视皮肤入射剂量率>100 mGy/min有24例,占35%.②冠脉造影+介入(I+CA)44例,总累积剂量面积乘积均值为(20 811.8±11 704.1)μGy/m2;总累积皮肤入射剂量均值为(2979.7±1625.1)mGy,其中有42例>2 Gy,占95%;透视皮肤入射剂量率>100 mGy/min为33例,占75%.结论 I+CA被检者所受辐射剂量约是CA的4.4倍.CA和I+CA的透视皮肤入射剂量率超过指导水平的分别占到35%和75%;总累积皮肤入射剂量>2 Gy者分别占到9%和95%,提示我们在医用放射诊疗防护中介入放射学的防护是重中之重,必须建立一套严格有效的管理机制. Objective To investigate how to reduce the risk of radiation therapy and to avoid the occurrence of certainty effect by using the statistic, analysis and evaluation of the radiation dose of the coronary angiography and interventional therapy. Methods The total radiation dose was collected and its related parameters in 113 cases of the examination were analyzed. Results (1)Simple coronary angiography (CA) in 69 cases. The total cumulative dose area product(DAP) mean was (7257.8±7935.6)μGy/m2; total cumulative skin entrance dose (ESD) was (436.00±418.87)mGy, including 6 cases〉2 gy, accounting for 9%; perspective skin entrance dose rate〉100 mGy/min was in 24 patients, accounting for 35%. (2)Coronary angiography and interventional(1+CA) 44 cases, the total cumulative dose area product(DAP ) mean(20 811.8±11 704.1 )μGy/m2; total cumulative skin dose (ESD), and the mean value was (2979.7±1625.1)mGy, which had 42 cases〉2 gy, accounting for 95%; perspective skin entrance dose rate〉100 mGy/min in 33 cases, accounting for 75%. Conclusion The total cumulative skin entrances are 9% and 95% indicating us the importance of radiation protection in the medical radiological diagnosis, we must establish a set of strict and effective management mechanism.
