摘要
目的初步探讨体素内不相干运动(intravoxel incohere motion,IVIM)技术在短暂性脑缺血发作(transient ischemic attack,TIA)中的应用价值。方法收集19例经临床诊断为TIA的患者及19例健康志愿者(对照组)作为研究对象,所有病例均进行MRI弥散加权成像(diffusion weighted imaging,DWI)及IVIM扫描。所得数据经过后处理得到标准弥散系数(standard apparentdiffusion coefficient,standard ADC)、慢扩散系数(slow apparentdiffusion coefficient,slow ADC)、快弥散系数(fast apparentdiffusion coefficient,fast ADC)。分别测量TIA组及对照组两侧大脑半球的standard ADC值、slow ADC值及fast ADC值。采用两独立样本t检验进行统计学分析。结果 19例TIA患者随访MRI检查结果显示,8例患者DWI序列显示进展为新发脑梗死。与对照组比较,TIA组fast ADC值降低,两者差异有统计学意义(P<0.01),standard ADC值、slow ADC值差异无统计学意义。结论 IVIM成像能够反映弥散信息及局部脑组织微循环灌注的变化,有望为TIA的早期诊断提供一种新的影像学检查方法。
Objective To evaluate the potential utility of intravoxel incoherent motion( IVIM) in transient ischemic attack( TIA). Methods The subject consisted of 40 patients diagnosed as transient ischemic attack and 30 cases of healthy volunteers as control group. All cases performed MRI diffusion weighted imaging( DWI) and IVIM scanning. Three parameters were derived from IVIM through image processing,namely standard apparent diffusion coefficient( standard ADC),slow apparent diffusion coefficient( slow ADC) and fast apparent diffusion coefficient( fast ADC). The IVIM parameters of bilateral cerebral hemisphere in TIA group and the control group were measured respectively and analyzed by two independent sample t tests. Results In TIA group,high signals were shown in initial DWI sequences of twenty-one patients. Among nineteen cases with normal initial DWI sequences,eight patients were diagnosed as new cerebral infarction in follow-up MRI examination. The fast ADC values in TIA group were significantly decreased than those in control group( P〈0. 01),while there were no significantly differences in standard ADC and slow ADC between two groups( P〉0. 05). Conclusions IVIM imaging shows diffuse information and changes of microcirculation perfusion in local cerebral tissue,which is expected to improve the early diagnosis of TIA.
