摘要
目的探究厄贝沙坦联合阿托伐他汀对老年高血压患者血清炎症因子及心功能的影响。方法选取温州医科大学附属第三医院心内科2013年1月-2015年6月收治的160例老年高血压合并左心室肥厚患者,随机分成2组。对照组患者给予厄贝沙坦,观察组给予厄贝沙坦联合阿托伐他汀。比较2组患者治疗前后基本生化指标、收缩压和舒张压、血清中C反应蛋白（CRP）、白细胞介素17（IL-17）和白细胞介素18（IL-18）含量,并测定患者心脏功能相关参数。结果治疗后观察组患者尿白蛋白排泄率（urinary albumin excretion rate,UAER）、甘油三酯（triglyceride,TG）、总胆固醇（total cholesterol,TC）、低密度脂蛋白（low density lipoprotein,LDL）、血肌酐（serum creatinine,Scr）显著低于对照组（P〈0.05）。治疗后观察组患者CRP、IL-17和IL-18显著低于对照组（P〈0.05）。治疗后2组血压相比差异无统计学意义。治疗后观察组左室重量指数（left ventricular mass index,LVMI）显著低于对照组,而室间隔舒张末期厚度（left ventricular septum thickness,LVST）显著高于对照组（P〈0.05）。结论厄贝沙坦联合阿托伐他汀治疗老年高血压患者效果显著,可显著抑制炎症,改善心功能。
Objective To explore the effect of irbesartan combined with atorvastatin on inflammatory factors and cardiac function in elderly patients with hypertension. Methods 160 cases of elderly hypertensive patients selected from January 2013 to June 2015 were were randomly divided into two groups. The control group received irbesartan treatment,and observation group received atorvastatin treatment on the basisi of control group. The biochemical indicators,blood pressure,contents of CRP,IL-17,IL-18 and cardiac function were compared between two groups of patients before and after treatment. Results After treatment,urinary albumin excretion rate（UAER）,triglyceride（TG）,total cholesterol（TC）,and low density lipoprotein（LDL）,serum creatinine（Scr） in observation group were significantly lower than control group（P 〈0. 05）. The CRP,IL-17 and IL-18 in observation group were significantly lower than control group（P 〈0. 05）. There was no significant difference in blood pressure between two groups. Compared with control group,the left ventricular mass index（LVMI） in observation group was lower and left ventricular septum thickness（LVST） was higher（P 〈0. 05）. Conclusion Irbesartan combined with atorvastatin therapy has an obvious effect in the treatment of elderly patients with hypertension,could significantly reduce inflammation,improve heart function.
