摘要
目的 探讨胃肠道间质瘤(gastrointestinal stromal tumors,GIST)患者循环肿瘤细胞的监测及其对预后的意义.方法 选择2011-2015年在南通大学附属医院接受治疗的58例GIST患者,提取外周静脉血中单核细胞RNA并反转录成cDNA,用实时定量PCR法检测DOG1基因的表达,并根据危险度分级,每1~3个月检测1次.结果 正常人外周血DOG1最高表达量为3×10-5,58例GIST患者的循环肿瘤细胞阳性率达到52%.循环肿瘤细胞与肿瘤大小(x2=17.872,P=0.000)和核分裂象(x2=10.945,P=0.011)密切相关,其监测复发或转移结果与CT结果一致性较好(kappa=0.685,P=0.000).循环肿瘤细胞阳性患者无病生存期短于阴性患者(12.4个月比28.7个月,P=0.038).结论 GIST患者的循环肿瘤细胞可以作为一个动态监测病情的指标.
Objective To evaluate monitoring circulating tumor cells (CTC) and the prognosis in gastrointestinal stromal tumors (GIST).Method 10 ml peripheral blood of GIST patients were collected.The RNA was extracted from peripheral blood mononuclear cells and then transcribed into cDNA,quantitative real-time PCR (qRT-PCR) was performed to detect the expression of DOG1 for one time every 1 to 3 months according to risk level.Results 52% of 58 GSIT patients were found CTC positive.CTC positive was correlated with tumor size (x2 =17.872,P =0.000) and mitotic count (x2 =10.945,P =0.011).CTC monitoring relapse was consistent with computed tomography (Kappa =0.685,P =0.000).CTC positive predicts poor disease-free survival (12.4 vs.28.7 months,P =0.038).Conclusion Detection of CTC based on qRT-PCR serves as a dynamic way to monitor the recurrence and metastasis of GIST patients.
