摘要 目的探讨18F脱氧葡萄糖(18F-FDG)PET-CT显像对肺癌TNM分期的诊断价值。方法回顾性分析行PET-CT全身检查并最终病理确诊的肺癌患者52例病例资料。参照最新TNM分期,判定PET-CT对临床分期的影响,再与病理分期相比较。两种分期进行一致性比较采用Kappa检验,各分期诊断准确率的比较采用χ2检验。M分期经病理、临床治疗及影像随访证实。结果 52例肺癌患者中,PET-CT改变临床分期20例(20/52,38.4%),其中升高10例(10/52,19.2%),降低10例(10/52,19.2%)。行手术治疗的40例患者中,PET-CT分期(cT NM)与病理分期(pT NM)比较,Ⅰ、Ⅱ、Ⅲ及Ⅳ期诊断准确性分别为94.4%(17/18)、25.0%(1/4)、44.4%(4/9)及77.8%(7/9),其中Ⅰ期与Ⅱ、Ⅲ期差异均有统计学意义(P<0.005),cT NM与pT NM一致性尚可(Kappa=0.589),总符合率为72.5%。结论 PET-CT对早期肺癌及远处转移具有较高的诊断价值,有助于肺癌的早诊断、早治疗以及判断预后,对制定治疗方案提供一定的临床依据。 Objective To explore the diagnostic value of 18 F-FDG PET-CT imaging in lung cancer staging.Methods We analyzed ret-rospectively data of 52 patients who underwent PET-CT imaging in Anhui Provincial Hospital from September 2013 to December 2015 and were finally diagnosed lung cancer.According to the latest TNMstaging,we estimated the influence of PET-CT on lung cancer clini-cal staging,and compared it with the pathological staging.In terms of consistency of two kinds of staging,we used Kappa value.When it comes to the comparison of diagnostic accuracy rate in staging,we used Chi-squared texting.M staging was confirmed from pathology, clinical treatment and follow-up imaging.Results Among the 52 cases of patients with lung cancer,PET-CT resulted in changes in 20 cases’clinical staging (20 /52,38.46%),among which 10 cases of staging increased (10 /52,19.23%)and 10 cases of staging re-duced(10 /52,19.23%).Among the 40 cases of surgical treatment,we compared the PET-CT staging (cTNM)with pathological stag-ing (pTNM)and the diagnostic accuracy rates of four stages from Ⅰ to Ⅳ were 94.44% (17 /18),25.00% (1 /4),44.4% (4 /9) and 77.78% (7 /9).Among the four stages,there were significant differences between stage Ⅰand stage Ⅱ and Ⅲ (P 〈0.005),and c-TNMstaging and p-TNMstaging had an acceptable consistency (Kappa value =0.589).The total coincidence rate was 72.5%.Con-clusions 18 F-FDG PET-CT has a higher diagnostic value for early lung cancer and distant metastasis,which is helpful in early diagno-sis,early treatment and prognosis of lung cancer,and also has a certain application value in clinical treatment.
