不同体重标准选择喉罩型号在全麻中的应用效果观察 被引量:2

摘要 目的比较不同体重标准选择喉罩型号在全麻中的应用效果。方法选择2013年1月-2015年12月在我院择期行喉罩全麻的平卧位手术患者180例,采用随机数字的方法分为A组(n=90例)和B组(n=90例)。A组按患者的实际体重选择喉罩型号,B组按标准体重选择喉罩型号。监测BP,HR,SpO_2,VT,PetCO_2,Ppeak,套囊内压力(CIP)。比较2组患者喉罩一次性置入成功率,置入时间,拔管后喉罩表面有无血迹及术后咽痛的发生率。结果 A组一次性置入成功率为72.2%,B组成功率为88.9%(P<0.05);A组置入时间明显长于B组,分别为(25±7)S与(19±5)S(P<0.05);术后拔管喉罩有血迹分别为16例(17.8%)和4例(4.4%)(P<0.05);术后咽痛分别为13例(14.4%)和3例(3.3%)(P<0.05)。B组置入成功指标均优于A组,B组术后并发症也少于A组,且具有统计学意义。结论依据患者标准体重选择喉罩型号一次性置入的成功率更高,口咽部损伤较轻。 Objective The objective of this study is to evaluate the feasibility of different weight standards in choosing the size of LMA in general anesthesia. Methods 180 patients underwent general anesthesia from January 2013 to December 2015 in our hospital. All of them were in the supine position during operation. We randomized these patients into group A(n=90) and group B(n=90). In group A we chose the size of LMA according with the actual weight,and in group B, we chose the size by the standard body weight.We monitored the BP,HR,SpO2,VT, PetCO2, Ppeak and CIP during the operation and anesthesia.Then to compare the success rate of inserting, the time of inserting,the incidence rate of post operation pharyngodynia and palatum durum bleeding between two groups.Results The one-time successful rate of inserting in group A was 72.2%, and the rate in group B was 88.9% (P〈 0.05) . We cost more time in group A than group B, the numbers were (25 ±7) s and (19±5) s (P〈0.05) differently. In group A there were 16 patients(14.4%) found of post operation palatum durum bleeding, and in group B the number were only 3 (3.3%)(P〈 0.05) .Conclusion We would have higher of LMA according to the standard body weight.The patients would also success rate of inserting when we select the size undergo less post operation complications.
