摘要
目的探讨特发性血小板减少性紫癜采用腹腔镜下脾切除术治疗的效果。方法选取我院2010年1月~2015年1月收治的40例特发性血小板减少性紫癜患者作为研究对象,随机分为试验组和对照组,各20例。对照组采用传统脾切除术进行治疗,试验组采用腹腔镜下脾切除术进行治疗,比较两组的治疗效果、术中出血量、手术时间、住院时间、复发率及并发症发生率。结果两组的总有效率比较,差异无统计学意义（P〉0.05）。试验组的术中出血量显著少于对照组,住院时间显著短于对照组,术后补液总量与住院总费用显著少于对照组,差异有统计学意义（P〈0.05）。两组的手术时间比较,差异无统计学意义（P〉0.05）。两组的复发率比较,差异无统计学意义（P〉0.05）。试验组的并发症发生率为15.0%,显著低于对照组的40.0%,差异无统计学意义（P〉0.05）。结论采用腹腔镜下脾切除术治疗特发性血小板减少性紫癜可以有效改善患者的症状,手术安全,复发率低,并发症少,具有重要临床意义。
Objective To explore the effect of laparoscopic splenectomy in the treatment of idiopathic thrombocytopenic purpura.Methods 40 patients with idiopathic thrombocytopenic purpura from January 2010 to January 2015 in our hospital were selected and randomly divided into the trial group and the control group,20 cases in each group.The control group was treated with traditional splenectomy,the trial group was treated with laparoscopic splenectomy.The therapeutic effect,the amount of intraoperative blood loss,operative time,length of hospital stay,recurrence rate and the incidence rate of complication in the two groups was compared.Results There was no significant difference in the total effective rate between the two groups（P〉0.05）.The amount of intraoperative blood loss in the trial group was less than that in the control group,the length of hospital stay in the trial group was shorter than that in the control group,the total amount of fluid infusion and the total hospital costs in the trial group was less than that in the control group,with significant difference（P〈0.05）.There was no significant difference in the operative time between the two groups（P〉0.05）.There was no significant difference in the recurrence rate between the two groups（P〉0.05）.The incidence rate of complication in the trial group was 15.0%,which was lower than 40.0% in the control group,with significant difference（P〈0.05）.Conclusion Laparoscopic splenectomy in the treatment of idiopathic thrombocytopenic purpura can effectively improve the patient′s symptoms,operation is safe,with low recurrence rate,fewer complications,it has important clinical significance.
