5-氨基酮戊酸光动力疗法联合激光对尖锐湿疣效果 被引量:5

摘要 目的观察5-氨基酮戊酸光动力疗法(ALA-PDT)联合二氧化碳(CO_2)激光治疗尖锐湿疣的临床效果。方法将219例尖锐湿疣病人随机分为治疗组112例和对照组107例,均采用CO_2激光去除可见疣体。然后治疗组应用200g/L的5-ALA溶液局部湿敷,封包避光3~4h,最后应用XD-635AB型光动力治疗仪照射,功率260~300mW,照射剂量100J/cm^2,每次20min,每周1次,连续治疗4次。对照组局部外用干扰素凝胶,每天4次,共治疗4周。治疗后1、2、3及6个月时分别评价疗效。结果随访3个月后,治疗组97例临床治愈,治愈率为86.6%;对照组69例临床治愈,治愈率为64.5%,两组比较差异有显著性(χ2=14.597,P<0.05);随访6个月后,治疗组95例治愈,治愈率为84.8%;对照组66例治愈,治愈率为61.7%,两组比较差异有显著性(χ~2=15.048,P<0.05)。结论 ALA-PDT联合CO_2激光治疗尖锐湿疣效果确切,值得临床推广应用。 Objective To observe the clinical efficacy of 5-aminolevulinic acid(ALA)-photodynamic therapy(PDT)combined with CO2 laserin the treatment of condyloma acuminata(CA). Methods A total of 219 CA patients were enrolled in this study and randomized to treatment group(n=112)and control group(n=107).CO_2 laser was used to remove the visible warts in both groups.After removing the warts,the patients in treatment group received 200g/L 5-ALA solution for local wet compress,sealing and avoiding light for 3-4h,and then,XD-635 AB photodynamic therapy(PDT)was carried out-with the power of 260-300 mW,irradiation dose of 100J/cm^2,20 minutes each time,once a week for four weeks.Those in the control group received topical interferon gel,four times a day for four weeks.The efficacy was evaluated after one,two,three and six months of treatment. Results A three-month following up showed that 97 patients in the treatment group were clinically cured,cure rate being86.6%,and in control group,69 were cured,cure rate 64.5%,the difference in cure rate between the two groups was significant(χ~2=14.597,P〈0.05).A six-month follow-up showed 95(84.8%)in treatment group were cured and 66(61.7%)in control group were cured,the difference between the two groups was significant(χ~2=15.048,P〈0.05). Conclusion ALA-PDT combined with CO_2 laser is effective and safe in the treatment of condyloma acuminata and worthy of clinical popularization and application.
