超声造影伽马拟合时间-强度曲线在结节性甲状腺肿中的诊断价值 被引量:1

摘要 目的探讨结节性甲状腺肿超声造影模式,分析伽马拟合时间-强度曲线在结节性甲状腺肿中的诊断价值。方法选择80例结节性甲状腺肿患者,其中男性47例,女性33例;年龄19~56岁,平均年龄38.70岁。采用Logiq E9彩色多普勒超声诊断仪行超声造影,回顾性分析其用伽马拟合时间-强度曲线,对瘤体达峰时间(TtoPk)、到达时间(ATM)、梯度(Grad)、曲线下面积(Area)及峰值强度各参数进行对比。结果肿块与周围正常甲状腺组织呈等增强53例(66.3%),无增强12例(15.0%),不均匀低增强10例(12.5%),高增强5例(6.3%)。TtoPk、ATM、Grad、Area与周围甲状腺组织相比,差异无统计学意义(P〉0.05)。峰值强度与周围甲状腺组织相比,差异有统计学意义(P〈0.05)。结论结节性甲状腺肿的造影模式呈现多样性,但等增强仍然是其主要增强模式,且结节内部造影峰值强度略高于周围正常组织。 Objective To investigate performance of nodular goiter with contrast-enhanced ultrasound(CEUS) and analyze diagnostic value of time-intensity curve(TIC) of gamma fitting in nodular goiter. Methods A total of 80 patients with nodular goiter were enrolled, which included 47 males and 33 females, aged 19- 56 years old with mean age of 38.70 years old. The CEUS was performed by Logiq E9 and the parameters of tumor time to peak(TtoPk), arrival time(ATM), gradient(Grad), area under curve(Area), peak intensity(PI) and TIC were compared by TIC of gamma fitting. Results In tumor and surrounding normal thyroid tissue, 53 cases(66.3 %) nodules showed isoenhancement, 12(15.0 %) of non-enhancement, 10(12.5 %) of hypoenhance-ment and 5(6.3 %) of hyperenhancement. Compared with surrounding thyroid tissue,there was no significant difference in TtoPk,ATM, Grad, Area(P〉0.05), but there was significantly difference in PI( P〈0.05). Conclusion It is demonstrated that the performance of nodular goiter with CEUS are diversity, isoenhancement is the major CEUS enhancement pattern, and PI of nodular goiter is higher than that of surrounding thyroid tissue.
