摘要
目的:检测SALL4在恶性生殖细胞肿瘤(malignant germ cell tumor,MGCT)中的表达,探讨其在MGCT病理诊断中的意义。方法:收集经手术切除、资料完整、来自性腺及性腺外的MGCT 80例作为研究组,其中无性细胞瘤22例,精原细胞瘤20例,卵黄囊瘤16例,未成熟性畸胎瘤12例,混合性生殖细胞肿瘤8例,胚胎性癌2例;选取同期手术切除的非生殖细胞性恶性肿瘤(No malignant germ cell tumor,NMGCT)标本40例作为对照组;应用常规病理学技术、免疫组化二步法增强型辣根酶聚合物(Polink-2HRP)染色系统,对全部120例肿瘤进行石蜡切片、HE染色和免疫组化SALL4标记,光学显微镜下观察评估。结果:MGCT的病理形态各有特点,免疫组化标记SALL4显色为核阳性,在80例MGCT中,73例为强阳性表达,未成熟性畸胎瘤有5例弱阳性、2例阴性,总计阳性率97.5%;对照组40例NMGCT中,5例弱阳性,余阴性,阳性率12.5%;经χ2检验P<0.05,差异具有显著意义。结论:在性腺及性腺外MGCT中,以免疫组化的方法,检测干细胞标记物SALL4的表达,证实SALL4在MGCT的病理诊断中的重要作用,可为MGCT的临床诊疗提供可靠依据。
Objectives: To study the expression of Sal-like protein4( SALL4) in malignant germ cell tumor( MGCT) and its significance in MGCT pathological diagnosis. Methods: There were 80 cases of MGCT samples from the First Affiliated Hospital of Gannan Medical University,including 22 cases of dysgerminomas,20 cases of seminomas,16 cases of yolk sac tumors,12 cases of immature teratomas,8 cases of mixed germ cell tumors,and 2 cases of embryonal carcinomas,These samples were collected from January 2009 to December 2014. Fourty cases of nonmalignant germ cell tumors( NMGCT),which were removed from female bilateral ovaries in homochronous operations,were taken as the controls.Conventional pathology techniques as well as Polink-2HRP were used for paraffin embedding,HE staining,and immunohistochemistry staining of SALL4 for all 120 samples. The tissues were observed under optical microscopes. Results: Various pathological characterizations were observed for MGCT,and immunohistochemistry staining of SALL4 was positive in nucleus. In 80 cases of MGCT,73 cases showed strongly positive in SALL4. There were 5 weak positive and 2 negative cases,and the total positive rate was 97. 5% for immature MGCT. For 40 cases of controls,5 cases were positive with weak intensity,and the rest were negative,and the positive rate was 12. 5%. Under Chi-square test,the differences of SALL4 positive rates between controls and MGCT group was statistically significant( P〈0. 05). Conclusion: For the MGCT inside and outside of gonad,the immunohistochemistry staining of stem cell marker SALL the importance of SALL4 in the MGCT pathological diagnosis,which could provide reliable foundation for MGCT clinical diagnosis and treatment.
