摘要
目的调查新诊断标准下张家港地区妊娠期糖尿病(GDM)发病率及高危因素。方法采用调查问卷的方法,对2014年4月-11月在张家港地区建卡并按规定进行围产保健的2 795例孕妇发放调查问卷进行调查。选出符合GDM诊断标准的孕妇作为观察组,非GDM孕妇为对照组。采用SPSS 16.0统计软件分析结果。结果 1新诊断标准下张家港地区GDM发病率为19.57%。2单因素情况比较结果显示:多囊卵巢综合征(PCOS)病史、孕前体重指数(BMI)、妊娠年龄、糖尿病家族史、孕次≥3次、流产≥3次、巨大儿史、剖宫产史、既往GDM病史、早孕建卡时空腹血糖及建卡时舒张压≥80 mm Hg是新诊断标准下GDM的独立危险因素。3多因素Logistic回归结果显示:妊娠年龄、糖尿病家族史、孕次≥3次、既往GDM病史、早孕建卡时空腹血糖及建卡时舒张压≥80 mm Hg是新诊断标准下GDM的独立危险因素。结论新诊断标准下张家港地区GDM发生率较高。孕时年龄、孕前体重指数、孕期血压、糖尿病家族史、曾患GDM病史、孕早期空腹血糖及孕次是GDM的高危因素。
Objective To investigate the incidence rate and high risk factors of gestational diabetes mellitus (GDM) under the new di- agnostic criteria in Zhangjiagang area. Methods A questionnaire survey study was conducted among 2 795 pregnant women receiving peri- natal health care in Zhangjiagang area from April to November in 2014, the pregnant women meeting the diagnostic criteria of GDM were se- lected as observation group, and the pregnant women without GDM were seiected as control group. SPSS 16. 0 software was used for statistical analysis. Results The incidence rate of GDM under the new diagnostic criteria in Zhangjiagang area was 19. 57%. Univariate analysis showed that polycystic ovary syndrome (PCOS) history, prenatal body mass iridex (BMI), maternal age, family history of diabetes, gravidi ty ≥ 3, abortion times ≥ 3, macrosomia history, cesarean section history, GDM history, the fasting blood glucose of early pregnancy examina- tion and diastolic blood pressure ≥80 mmHg of early pregnancy examination were independent high risk factors of GDM. Multivariate logistic regression analysis showed that maternal age, family history of diabetes, gravidity ≥3, GDM history, the fasting blood glucose of early preg- nancy examination and diastolic blood pressure ≥ 80 mmHg of early pregnancy examination were independent high risk factors of GDM. Con- clusion The incidence rate of GDM under the new diagnostic criteria in Zhangjiagang area is relatively high. Maternal age, prenatal BMI, blood pressure during pregnancy, family history of diabetes, GDM history, the fasting blood glucose during early pregnancy, and gravidity are high risk factors of GDM.
