摘要目的综合分析腹腔高压（intra—abdominal hypertension）伴急性呼吸窘迫综合征（acute respiratory distress syndrome，ARDS）行机械通气患者的护理效果，为腹腔高压伴ARDS患者的治疗提供科学的数据参考。方法选择我院2014年6月～2015年7月收治的61例机械通气患者，采用针对性的护理措施。分析患者护理前后中心静脉压、PaCO2、SaO2、PaCO2/FIO2等指标参数情况。结果护理前，患者的中心静脉压、PaCO2、SaO2、PaCO2／FIO2等指标参数分别为（8．87±2．68）cmH2O、（94．88±9．01）mmHg、（88．79±4．06）％、（135．26±21．01）mmHg；护理后，患者的中心静脉压、PaCO2、SaO2、PaCO2/FIO2等指标参数分别为（7．68±1．84）cmH2O、（68．25±8．47）mmHg、（95．32±3．77）％、（261．11±22．03）mmHg，各指标参数显著优于护理前（P〈0.05）。结论应用针对性的护理措施能够显著改善腹腔高压伴ARDS行机械通气患者的中心静脉压、PaCO2、SaO2、PaCO2/FIO2指标，提高护理效果，具有极高的临床应用价值。 Objective To comprehensively analyze the nursing effects of the patients with intra-abdominal hypertension complicated with ARDS （acute respiratory distress syndrome） receiving mechanical ventilation, so as to provide references of scientific data in the treatment of intra-abdominal hypertension complicated with ARDS. Methods Clinical data of 61 patients who were admitted to our hospital and were given mechanical ventilation from June 2014 to July 2015 were selected as research subjects. Targeted nursing measures were applied for them for related nursing. Central venous pressure, PaCO2, SaO2, PaCO2/FIO2 and other indicators were compared before and after the nursing of patients with intra-abdominal hypertension complicated with ARDS. Results Before the nursing, central venous pressure, PaCO2, SaO2, PaCO2/FIO2 were（8.87±2.68） cmH2O, （94.88±9.01） mmHg, （88.79±4.06）%, （135.26±21.01） mmHg respectively; after the nursing, central venous pressure, PaCO2, SaO2, PaCO2/FIO2 were （7.68±1.84） emH2O, （68.25±8.47） mmHg, （95.32±3.77）%, （261.11±22.03） mmHg respectively. After the nursing, central venous pressure, PaCO2, SaO2, PaCO2/FIO2 and other indicators were significantly better than those before nursing （P〈0.05）. Conclusion Application of targeted nursing measures is able to significantly improve the index of central venous pressure, PaCO2, SaO2, PaCO2/FIO2 and the nursing effects of patients with intra-abdominal hypertension complicated with ARDS receiving mechanical ventilation, which is of high clinical application value.
