摘要
目的通过研究慢性自发性荨麻疹(CSU)患者与健康对照组血清脱氢表雄酮硫酸酯(DHEA-S)及总IgE,IL-2,IL-4,IFN-γ水平的差异,探讨CSU患者外周血DHEA-S是否与总IgE、Th1/Th2细胞失衡相关。方法 162例CSU患者为实验组,68例健康人为对照组。应用放射免疫分析法测定血清中DHEA-S水平;采用ELISA法检测外周血总IgE,IL-2,IL-4,IFN-γ水平。采用t检验和直线相关性分析分析各组检测值。结果 CSU组外周血DHEA-S,IL-2,IFN-γ水平明显低于健康对照组,血清总IgE,IL-4水平明显高于健康对照组,差异有统计学意义(P<0.05)。CSU病情分级与DHEA-S水平呈负相关,与总IgE水平呈正相关(r=-0.664,-0.419,P均<0.01),与IL-2,IL-4,IFN-γ水平无明显相关性。CSU组DHEA-S与IL-2,IFN-γ呈显著正相关,与IgE,IL-4呈显著负相关(r=0.409,0.397,-0.319,-0.367,P均<0.001)。结论 CSU患者体内可能存在内分泌免疫系统失衡,外周血DHEA-S水平下降可能与总IgE水平升高及Th1/Th2细胞失衡相关。
Objective To investigate chronic spontaneous urticaria's (CSU) dehydroepiandrosterone sulfate relation to total serum IgE, IL-2, IL-4, and IFN-γ in peripheral blood, and further to investigate chronic urticaria's dehydroepiandrosterone sulfate relation to total serum IgE and Th1/Th2 out of balance. Methods The serum ehydroepiandrosterone sulfate had been measured by RIA. The expression of total serum IgE, IL-2, IL-4, IFN-γ had been measured by ELISA in 162 patients with CSU, 68 normal controls. All groups of date were analysised by t test and linear correlation. Results The expression of DHEA-S, IL-2, IFN-γ were lower in patients with CSU than in the normal controls ( P 〈 0. 05 ). The expression of total serum IgE, IL-4 were higher in patients with CSU than in the normal controls ( P 〈 0. 05 ). The disease classifiction was negative correlation with serum DHEA-S, and it was also positively correlated with the serum total IgE ( r = - 0. 664, -0. 419, all P 〈 0. 01 ) , but the expression of IL-2, IL-4, IFN-γ had no significant correlation with the disease classification. The expression of DHEA-S in CSU patient's was positive correlation to the level of IL-2 and IFN-γ,but it was negative correlation to the level of total serum IgE and IL-4 ( r = 0. 409,0. 397, - 0. 319, -0. 367, all P 〈0. 001 ). Conclusion The neuroendocrine-immune system dysfunction might be related to the CSU. The expression of serum DHEA-S might be relation to total IgE and Th1/Th2 out of balance.
