摘要
目的 研究分析全髋关节置换术中不同比重盐酸罗哌卡因蛛网膜下腔麻醉的临床效果。方法 选取在我院治疗的全髋关节置换术患者72例（2013年3月~2015年9月）。将其动态随机化分3组,每组各24例。分别给予不同比重盐酸罗哌卡因对Ⅰ组、Ⅱ组、Ⅲ组进行麻醉,对比3组患者运动、感觉阻滞起效时间及不良反应发生情况及记录麻醉效果。结果 观察3组全髋关节置换术患者运动、感觉阻滞起效时间发现,Ⅰ组〉Ⅱ组〉Ⅲ组,对比3组运动、感觉阻滞起效时间存在明显差异（P〈0.05）;Ⅲ组不良反应发生率（29.17%）〉Ⅰ组不良反应发生率（16.67%）〉Ⅱ组不良反应发生率（12.50%）,Ⅱ组Ⅲ组麻醉效果相仿但明显好于I组。比较发现Ⅱ组数据更有优势（P〈0.05）。结论 对全髋关节置换术患者采用Ⅱ组盐酸罗哌卡因进行麻醉较Ⅰ组盐酸罗哌卡因效果更显著,Ⅱ组盐酸罗哌卡因麻醉不良反应更少,在临床上可以广泛应用。
Objective To study the clinical effect of total hip arthroplasty in different proportion of ropivacaine subarachnoid anesthesia.Methods72 cases of total hip replacement in our hospital were selected （March 2013- September 2015）. The dynamic randomization was divided into 3 groups, 24 cases in each group. 3 groups were treated with different proportion of ropivacaine hydrochloride, and three groups of patients with movement, sensory block onset time and adverse reactions occurrence records and anesthetic effect were compared.Results Observation group 3 total hip arthroplasty in patients with movement, sensory block onset time was found, groupⅠ〉group Ⅱ〉groupⅢ, comparing three groups of movement, feeling block onset time, there was significant difference （P〈0.05）; the incidence of adverse reaction in groupⅢrate （29.17%）〉groupⅠ of adverse reactions occur rate （16.67%） 〉groupⅡ adverse reactions occur rate （12.50%）, groupⅡ,Ⅲgroup, the anesthesia effect of similar but signiifcantly better than that in group Ⅰ. The comparison found that group II data was more advantageous （P〈0.05）. Conclusion In patients with total hip arthroplasty by group Ⅱ ropivacaine hydrochloride anesthesia than groupⅠropivacaine hydrochloride effect more signiifcant, groupⅡ ropivacaine hydrochloride anesthesia fewer adverse reactions, can be widely used in clinic.
