摘要
目的探讨卵巢Brenner瘤各类亚型的影像表现,以提高对该病的认识及诊断的准确性。方法回顾分析15例经手术病理证实的卵巢Brenner瘤患者的CT及MR资料,包括良性Brenner瘤9例,交界性Brenner瘤3例,恶性Brenner瘤3例。结果良性及恶性Brenner瘤以囊实性为主,良性Brenner瘤以单囊为主,均匀的实性成分多位于肿瘤的边缘,并与肌肉的密度或信号相仿,增强后呈轻中度强化,多见钙化,且钙化多位于实性成分边缘;恶性Brenner瘤为不均匀的实性成分中掺杂着多个小囊,实性成分的T2WI信号高于周围肌肉组织,增强后强化曲线呈速升平台型,少见播撒转移,多直接侵犯临近脏器;交界性Brenner瘤则以多囊分隔为主,分隔厚薄不均,部分有附壁结节;所有Brenner瘤均未见明显腹水。结论良恶性及交界性Brenner瘤的影像表现各有特征性,与其他卵巢肿瘤鉴别时需具体分析不同亚型Brenner瘤的影像特征。
Objective To investigate the characteristics of ovarian Brenner tumor in CT and MRI and as a consequence to improve the accuracy of Brenner tumor diagnosis. Methods CT and MR images of 15 patients with histopathologically confirmed ovarian Brenner tumor (9 benign Brenner, 3 borderline Brenner, 3 malignant Brenner) were retrospectively reviewed. Result All Benign and malignant Brenner were a mixture of cystic and solid masses. Most of the benign tumors appeared as unilocular cyst with homogeneous eccentric solid portion. The solid partappeared to be isodensity or isointensity to that of the adjacent muscles, and showed mild to moderate enhancement in images. The calcification was seen at the edge of the solid portion in the most cases; malignant Brenner tumors appeared as non-uniform solid mass mixed with little multilocular cyst, and the signal intensity of solid components on T2-weighted MR images was hyperintense compared with that of muscle, the time-signal curve of DCE-MRI for malignant Brenner was fast rise platform type. Some of the malignant Brenner tumors presented perihepatic seeding metastasis, whereas most of the malignant Brenner showed local direct invasion into adjacent organ (e.g. uterus or sigmoid colon); borderline Brenner appeared as multilocular cystic with uneven thickness septa or mural nodule. None of the Brenner tummor showed massive ascites. Conclusion The CT and MR appearance of Brenner tumors varied according to tumor subtype, thus we should consider the imaging characteristics of different tumor subtype of Brenner tumor when differentiating them from other tumors in ovary.
